摘要
目的我们曾通过对实例心电图图谱的初步研究,证明了心电图"掉头"现象与心力衰竭存在密切关联。本阶段对该领域的研究进行完善与拓展,并尝试通过运用心电图的简单方法对既往遗留的复杂临床问题进行破解。方法通过收集2015年11月至2017年1月我院收治的有胸闷、气喘症状的8例心力衰竭患者新的心电图图谱,结合"掉头"现象,深入探讨心力衰竭心电图的特点。结果新的心电图图谱表明心电图"掉头"现象判断心力衰竭具有较高的可靠性,可以帮助分析临床疑难病情,有助于澄清和解决有争议的临床难题。结论心电图"掉头"现象是对Zema提出的"r波递增不良"概念的延伸,是心电学领域的重要发现。心电图诊断心力衰竭具有重要的临床应用及推广价值。
Objective By preliminary investigation on electrocardiogram（ ECG） examples,we have proved the close connection between ＂U-turn＂phenomenon and heart failure. At this stage we plan to complete and develop the research in this field,and attempt to answer previously remained complex clinical problems with easy methods of ECG. Methods The new ECG patterns of 8 patients with heart failure suffering from chest distress or asthma were collected,and they had been treated from November 2015 to January 2017 in our hospital. The electrocardiographic features of heart failure were deeply explored in combination with the ＂U-turn＂phenomenon. Results New ECG patterns illustrated that the ＂U-turn＂phenomenon on ECG is highly reliable in judging heart failure. It helps to judge difficult clinical conditions,and clarify and solve controversial clinical problems. Conclusion The＂U-turn＂phenomenon on ECG stretches from the concept of＂poor r wave growth＂proposed by Zema,which is an important finding in the field of electrocardiology. The utilization of ECG in diagnosing heart failure is worthy of being vigorously promoted.
