摘要
目的：探讨补阳还五汤加减联合头针治疗缺血性中风失语气虚血瘀证的疗效。方法：选取缺血性中风后失语症患者82例,按数字表法随机分为对照组41例和观察组41例。根据《中国脑血管病防治指南》对两组患者予常规干预;对照组采取舒尔氏刺激法行言语康复训练,30 min/次,3次/周;在对照组基础上观察组予补阳还五汤加减治疗,1剂/d,分早晚2次内服;头针治疗,1次/d,5次/周。两组均连续治疗30 d。比较两组神经功能缺损评分、言语功能、气虚血瘀证症状评分及临床疗效。结果：治疗后2周和4周,观察组的神经功能缺损评分显著低于对照组同时点（P〈0.01）。治疗后,观察组的CRRCAE各指标评分显著高于对照组,气虚血瘀证（面色白光白、气短乏力、自汗出、舌质黯）症状评分明显低于对照组（P〈0.01）。观察组的总有效率为90.24%,明显高于对照组（68.29%）（P〈0.05）。结论：在常规治疗基础上,补阳还五汤加减联合头针治疗缺血性中风失语气虚血瘀证,可明显改善患者的神经功能缺损、言语功能及中医临床症状,提高临床疗效,值得进一步研究。
Objective：To investigate the efficacy of Buyang Huanwu Decoction combined with scalp acupuncture in treating with aphasia after ischemic stroke with syndrome of Qi-deficiency and blood-stagnation. Methods：Eighty-two cases with aphasia after ischemic stroke cases in our hospital were divided into control group（41 cases） and observation group（41 cases） referring to table of random number method. Both groups were given with the conventional therapy according to China Cerebrovascular Disease Prevention and Treatment Guidelines. Control group was given with Schuell＇s language training,30 min/time and 3 times/week. On basis of the control group,the observation group was given Buyang Huanwu Decoction（ one dose/d and twice a day） combined with scalp acupuncture（ once a day and 5 times/week）. Both groups were observed for 30 days. Scores of neurologic impairment,speech function,symptoms of syndrome of Qi-deficiency and blood-stagnation and efficacy were compared between two groups. Results：After 2 and 4 weeks＇ treatment,the score of neurologic impairment of observation group was obviously lower than that of the control group at same time（ P〈0.01）. Scores of indexes of Chinese rehabilitation research center aphasia examination（ CRRCAE） of observation group were remarkably higher,while symptoms scores of syndrome of Qi-deficiency and blood-stagnation were lower than those of the control group（ P〈0.01）. Total efficiency rate of observation group was 90. 24%,which was superior to the control group＇s（68. 29%）（ P〈0.05）. Conclusion：On the basis of the conventional therapy,Buyang Huanwu Decoction combined with scalp acupuncture in treating with aphasia after ischemic stroke with syndrome of Qi-deficiency and blood-stagnation can improve neurologic impairment,speech function and clinical symptoms of traditional Chinese medicine,increase the efficacy and deserves further study.
