摘要
目的总结性分析日常工作中经常遇到的单纯血小板减少患者的病因,帮助临床医生对单纯血小板减少性疾病作出快速、准确的诊断和鉴别,使患者得到及时、有效的治疗。方法择取152例2016年10月20日至2017年12月30日来我院就诊的患者,均经血常规检测显示白细胞计数、红细胞计数无明显异常,仅血小板<80×109/L,进行外周血涂片、染色、镜检及骨髓穿刺细胞学检查。结果 152例单纯血小板减少患者中,特发性血小板减少性紫癜(ITP)占59.87%(91/152),嗜血细胞综合征占25.00%(38/152),假性血小板减少占3.95%(6/152),血小板减少伴增生性贫血占3.29%(5/152),巨核系增生减低性血小板减少占3.29%(5/152),其它感染性疾病致血小板减少占1.97%(3/152),脾脏轻度肿大致血小板减少占1.32%(2/152),酒精性肝硬化致血小板减少占0.66%(1/152),毒蜂蛰伤致血小板减少占0.66%(1/152)。结论引起单纯血小板减少的主要原因是ITP,其次是嗜血细胞综合征、假性血小板减少、血小板减少伴增生性贫血和巨核系增生减低性血小板减少等。
Objective To summarize and analyze the causes of patients with thrombocytopenia often encountered in daily work, and to help clinicians to quickly and accurately diagnose and identify simple thrombocytopenic diseases, so that patients receive timely and effective treatment. Methods A total of 152 patients who came to our hospital from October 20,2016 to December 30, 2017 were selected and examined by routine blood test which showed no abnormal in white blood cell count and red blood cell count, only platelets 80×109/L, they were subjected to peripheral blood smear, staining,microscopy and bone marrow aspiration cytology. Results Among 152 patients with simple thrombocytopenia, idiopathic thrombocytopenic purpura ITP accounted for 59.87% 91/152 , hemophagocytic syndrome accounted for 25.00% 38/152 ,pseudo-platelet reduction accounted for 3.95% 6/152 , thrombocytopenia with proliferative anemia accounted for 3.29% 5/152 , mega nuclear hyperplasia reduced thrombocytopenia accounted for 3.29% 5/152 , thrombocytopenia caused byother infectious diseases accounted for 1.97% 3/152 , thrombocytopenia caused by mild swelling of the spleen accounted for 1.32% 2/152 , thrombocytopenia caused by alcoholic cirrhosis accounts for 0.66% 1/152 and thrombocytopenia caused by poisonous bee stings accounted for 0.66% 1/152 . Conclusion The main cause of thrombocytopenia is ITP, followed by hemophagocytic syndrome, pseudo -platelet reduction, thrombocytopenia with proliferative anemia and mega nuclear hyperplasia reduced thrombocytopenia.
