摘要
目的:探讨老年慢性冠心病心力衰竭(心衰)与血清糖类抗原125(Carbohydrate antigen 125,CA125)的相关性。方法:随机选取心内科及老年病科住院的共296例,按照入选标准、排除标准及年龄、性别匹配后,共纳入228例研究对象,其中174例为老年慢性冠心病心衰患者为病例组,心功能Ⅰ~Ⅱ级60例,心功能Ⅲ级63例,心功能Ⅳ级51例;选择同期住院的心功能正常(非心衰)且年龄性别匹配的54例患者为对照组。同期检测两组的CA125、B型钠尿肽前体(NT-Pro BNP)、超敏C-反应蛋白(hs-CRP)及射血分数(EF)等。结果:老年慢性冠心病心衰组CA125、NTPro BNP、hs-CRP水平均高于对照组,且随心功能的加重而升高,差异均有统计学意义(P<0.01);老年慢性冠心病心衰组,CA125与NYHA心功能分级、NT-Pro BNP、hs CRP均呈正相关(r值分别为0.270、0.344、0.257、0.295,P<0.01),与LVEF呈负相关(r值为-0.260,P<0.001)。结论:老年慢性冠心病心力衰竭与血清CA125具有相关性,而且有助于早期诊断老年慢性冠心病心衰。
Objective To explore the correlation of the chronic heart of coronary heart disease failure and the serum Carbohydrate antigen 125（ Carbohydrate antigen 125,CA125） level in elderly patients. Method A total of 296 hospitalzed patients in cardiology and geriatrics department were rendomly selected. According to the admission criteria,exclusion criteria,age and sex matching,228 subjects were included in the study. 174 inpatients with chronic heart of coronary heart disease failure in the First Affiliated Hospital of School of medicine,Shihezi University were included randomly as case group,and 54 inpatients without chronic heart failure（ control group） entered the study. The CA125,precursor protein of B-type natriuretic peptide（ NT-pro BNP） and High sensitivity C reactive protein（ hs-CRP） and echocardiography data of all groups were determined in the same period,and the results were compared among different groups. Results CA125,NT-Pro BNP,hs CRP in the chronic heart of coronary heart disease failure patients were higher than the patients with control group,and increased gradually with the increase of cardiac function,the difference was statistically significant（ P 0. 01）; In elderly patients with the chronic heart of coronary heart disease failure patients,CA125 was positively correlated to NYHA heart function classification,NT-Pro BNP,hs CRP（ coefficients were 0. 270、0. 344、0. 257、0. 295 respectively,P 0. 01）; The LVEF and CA125 were negatively correlated（ r =-0. 260,P 0. 001）. Conclusion The chronic heart of coronary heart disease failure patients in elderly patients is correlation to the serum CA125 level,the serum CA125 level increased with the severity of heart failure,The reason may be related to inflammation factors.
