摘要
目的探究情景模拟案例训练对PKP治疗陈旧性及老年新鲜性脊柱骨折患者手术室护理效果的影响。方法选取本院自2014年4月—2017年4月期间收治的老年陈旧椎体压缩骨折及新鲜性椎体压缩骨折患者244例,均采用经皮椎体后凸成形术(PKP)治疗,按照就诊时间顺序分为对照组与观察组,每组各122例,对照组给予常规手术室护理,观察组在对照组基础上加用情景模拟案例训练,对比两组患者术前、术后2周及术后12个月疼痛程度、手术前后JOA腰椎功能评分及生活质量评分。结果两组患者术后2周、术后12个月相比于术前VAS评分均降低,术后12个月与术后2周相比VAS评分降低,观察组与对照组相比VAS评分较低,差异具有统计学意义(P<0.05)。两组术后3个月与术前相比JOA评分及生活质量评分均升高,观察组术后3个月与对照组术后3个月相比JOA评分及生活质量评分较高,差异具有统计学意义(P<0.05)。结论采用PKP治疗老年新鲜椎体压缩骨折及陈旧性骨折时在实施常规护理的基础上加用情景模拟案例法能够促进缓解疼痛,改善患者的肢体功能,促进提高生活质量。
Objective To explore the influence of situational simulation case training on the operating room nursing effect for patients suffered from obsolete or senile fresh spinal fractures treated by PKP.Metbods Totally 244 cases of elder patients suffered from obsolete or fresh vertebral compression fractures, those who hospitalized in our hospital during April 2014 and April 2017, were enrolled in the study. All the patients received the treatment of percutaneous kyphoplasty (PKP). According to the time sequence they were divided into control group and observation group, 122 cases in each group, control group was given routine operating room nursing, while the observation group was given total scenario simulation training on the basis of the control group. The ( 2 weeks and 12 months after the surgery) pain degree, preoperative and postoperative JOA lumbar function score and quality of life of the two groups were compared.Results Postoperative VAS scores (2 weeks and 12 months after the surgery) of the two groups decreased significantly compared with the preoperative and were lower, and the scores of 12 months after the surgery were significantly lower than those of 2 weeks after the surgery, and VAS score of observation group was significantly lower than the control group, the differences were statistically significant (P〈0.05). Three months after the surgery the JOA score and score of quality of life of the two groups increased compared with the preoperative scores, and the scores of the observation group was higher than those of the control group, the difference statistically significant (P〈 0. 05 ). Conclusions The application of situational simulation case training in the PKP treatment for patients suffered from obsolete or senile fresh spinal fractures could help releasing pain, improving the patient's limb function and promoting the quality of life.
