中药对自身免疫性重症肌无力动物干预作用实验研究的系统评价 被引量:9

Systematic Review of Animal Experiment Study on Chinese Medicine Intervention for Autoimmune Myasthenia Gravis

摘要 目的系统评价中药对自身免疫性重症肌无力动物的干预作用。方法检索中国知网、万方数据库、重庆维普中文科技期刊数据库、中国生物医学文献数据库、Pub Med、Cochrane Library和Embase,筛选中药干预自身免疫性重症肌无力的动物实验研究,制定资料提取表并提取资料,采用SYRCLE动物实验偏倚风险评估工具对纳入文献的研究方法学偏倚风险进行评估,并用Rev Man 5.3软件对结局指标[包括Lennon评分及血清乙酰胆碱受体抗体(AchR-Ab)、白细胞介素4(IL-4)、白细胞介素6(IL-6)、γ-干扰素(IFN-γ)、转化生长因子β(TGF-β)、肿瘤坏死因子α(TNF-α)]进行Meta分析。结果共纳入18项研究,研究对象为Lewis大鼠或SD大鼠或C57BL/6小鼠,均为随机对照研究,但偏倚风险较高。Meta分析结果显示,中药组可改善模型动物Lennon评分[SMD=-1.75,95%CI(-2.54,-0.96),P<0.0001],可降低血清AchR-Ab[SMD=-5.60,95%CI(-7.31,-3.88),P<0.0001)]、IL-4[SMD=-2.96,95%CI(-4.69,-1.23),P=0.0008]、IL-6[SMD=-3.65,95%CI(-5.01,-2.29),P<0.0001]、IFN-γ[SMD=-3.73,95%CI(-5.43,-2.03),P<0.0001]、TNF-α[SMD=-2.19,95%CI(-3.71,0.66),P=0.007]含量,提高血清TGF-β含量[SMD=4.43,95%CI(1.84,7.01),P=0.0008]。结论中药可调控免疫相关细胞因子的变化,改善自身免疫性重症肌无力动物症状。 Objective To systematically evaluate the intervention effect of Chinese medicine on autoimmune myasthenia gravis animals. Methods CNKI,Wanfang Database,Chongqing VIP Chinese Science and Technology Journal Database,China Biomedical Literature Database,Pub Med,Cochrane Library and Embase were selected to screen Chinese medicine for the treatment of autoimmune myasthenia gravis animals,and data extraction tables were formulated and the data were extracted. Risk assessment of the methodological bias of the included literature was performed by using the SYRCLE Animal Test Bias Risk Assessment Tool and the meta-analysis of outcome indicators [including Lennon score and serum acetylcholine receptor antibody(AchR-Ab),interleukin 4(IL-4),interleukin-6(IL-6),γ-interferon(IFN-γ),transforming growth factor β(TGF-β),and tumor necrosis factor α(TNF-α) ]was conducted with Rev Man 5. 3 software. Results A total of 18 studies were included in the study. The subkects were Lewis rats or SD rats or C57 BL/6 mice,and all the studies were randomized controlled trials. But the risk of bias was higher. Meta-analysis showed that the Chinese medicine group could improve the model animal Lennon score[SMD =-1. 75,95% CI(-2. 54,-0. 96),P〈0. 0001],reduce serum AchR-Ab [SMD =-5. 60,95% CI(-7. 31,-3. 88),P〈0. 0001) ],IL-4 [SMD =-2. 96,95% CI(-4. 69,-1. 23),P = 0. 0008],IL-6[SMD =-3. 65,95% CI(-5. 01,-2. 29),P〈0. 0001],IFN-γ [SMD =-3. 73,95% CI(-5. 43,-2. 03),P〈0. 0001],TNF-α [SMD =-2. 19,95% CI(-3. 71,0. 66),P = 0. 007] content,increased serum TGF-β content [SMD = 4. 43,95% CI(1. 84,7. 01),P = 0. 0008]. Conclusion Chinese medicine can regulate the changes of immune-related cytokines and improve symptoms of autoimmune myasthenia gravis animals.
