摘要
【摘要】目的讨论住院药师、临床药师、行政部门三级联合干预住院患者12服不合理医嘱的有效性。方法 对2017年1月至2017年6月住院药房登记的789例患者口服不合理医嘱的干预措施及有效性进行统计分析。结果住院药师电话通知的采纳率为80．10％，联合临床药师后采纳率为92．90％，行政部门参与后总采纳率为94．30％。结论三级联合干预措施，可以提高口服不合理医嘱的干预成功率，对促进临床合理用药有一定的促进作用。
Objective To investigate the effectiveness of oral unreasonable medical orders on hospitalized pharmacists, clinical pharmacists, and administrative departments for three-stage joint inter- ventions. Methods The intervention measures and effectiveness of oral unreasonable medical orders were analyzed statistically in 789 patients registered in inpatient dispensary from Jan 2017 to Jun 2017. Results The adoption rate of telephone notification by hospital pharmacists was 80. 10%, the adoption rate after combined clinical pharmacists was 92.90%, and the total acceptance rate after the participation of administrative departments was 94. 30%. Conclusions The three level joint intervention measures can increase the success rate of interventions with oral unreasonable medical orders and promote the rational use of drugs in clinics.
