摘要
目的研究出院后继续动机性访谈对康复期精神分裂症治疗依从性的影响。方法选取2014年3月~2017年3月入住我院的82例精神分裂症患者为研究对象,采用随机数字表法进行分组,实验组41例,对照组41例。两组患者在入院时均接受住院期间的动机性访谈及常规宣教,实验组则在出院后继续动机性访谈,对照组出院后则进行常规电话随访。分别在患者入院时、出院后1个月、3个月、6个月时完成患者的依从性资料收集。结果两组出院后1个月服药依从性差异无统计学意义(P>0.05)。出院后3个月和6个月时,实验组的服药依从率分别为92.68%和95.12%,明显高于同期的对照组(75.61%,70.73%)(P<0.05);实验组患者在出院后3个月和6个月时服药依从性得分则也显著高于对照组(P<0.05);在患者出院后参加社会活动、按时复诊、复发率、满意率四个方面实验组均显著优于对照组,差异有统计学意义(P<0.05)。结论出院后继续动机性访谈对康复期精神分裂症患者院外依从性的提高具有重要意义,具有针对性强、易于接受、适宜开展等特点,在提高院外精神分裂症患者依从性方面具有更好的远期效果。
Objective To explore the effect of continuing motivational interviewing after discharge to the treatment compliance of rehabilitation schizophrenia. Methods A total of 82 schizophrenia patients was selected from March 2014 to March 2017 in our hospital,and divided into the experimental group（41 cases）and the control group（41 cases）,using the random number table method. Both groups received the motivational interviewing conventional propaganda and education during the period of inpatient treatment. The experimental group continue motivational interviewing after discharge,while the control group was only given conventional telephone follow-up after discharge. Treatment compliance was evaluated at the time of admitted to hospital and 1 month,3 months,6 months after being discharged. Results There is no group difference for the medication adherence at one month after discharge（P〈0.05）. At 3 months and 6 months after discharge,the medication compliance rate in the experimental group（92.68% and 95.12%）was significantly higher than that of the same period in the control group（75.61%,70.73%）（P〈0.05）. Medication adherence score in the experimental group at 3 months and 6 months after discharge was also significantly higher than that of control group（P〈0.05）. To participate in social activities,timely follow-up after discharge from hospital,the recurrence rate,and satisfaction in the experimental group were significantly better than the that of control group（P〈0.05）. Conclusions Motivational interviewing is strong pertinence,easy to accept,and suitable to carry out. Continue motivational interviewing after discharge can significant improve the compliance of rehabilitation schizophrenia,and has better long-term outcome in improving the compliance of schizophrenia patients outside.
