强化肺部功能锻炼法在预防胸脊髓损伤并截瘫患者肺部感染的应用研究 被引量:10

摘要 目的观察强化肺部功能锻炼预防胸脊髓损伤并截瘫患者肺部感染的临床效果,以期降低胸脊髓损伤并截瘫患者肺部感染率。方法选择2015年2月-2017年1月于医院行胸脊髓损伤并截瘫患者108例为研究对象,采用数字随机表法将患者随机分为对照组和试验组,每组54例。对照组患者采用常规医护措施预防患者发生肺部感染,试验组患者在常规医护措施预防肺部感染的基础上,再予以肺部功能强化锻炼,比较两组患者入组后3个月肺部感染发生率、肺部功能指标氧分压(PaO_2)、氧合指数(PaO_2/FiO_2)、最大通气量MVV、第一秒用力呼气量(FEV1)变化、呼吸功能障碍率、呼吸功能分级评定的变化。结果入组后3个月,两组患者PaO_2、PaO_2/FiO_2、MVV、FEV1较入组前均升高(P<0.05),且试验组患者PaO_2、PaO_2/FiO_2、MVV、FEV1均高于对照组患者(P<0.05);3个月内试验组患者肺部感染率为3.70%,低于对照组患者的12.96%,但组间比较差异无统计学意义(P=0.060),呼吸功能障碍率为12.96%,低于对照组患者的24.07%(P=0.031);两组患者入组3个月后呼吸功能分级有所下降,试验组患者零~Ⅱ级例数多于对照组患者,但组间比较差异无统计学意义(P=0.161)。结论强化肺部功能锻炼可有效提升胸脊髓损伤合并截瘫患者呼吸功能,降低呼吸功能障碍率及肺部感染率,提升患者日常生活能力,对胸脊髓损伤合并截瘫患者具有较高的临床价值。 OBJECTIVE To observe the clinical effect of strengthening pulmonary function exercises on prevention of pulmonary infections in patients with thoracic spinal cord injury and paraplegia,so as to reduce the pulmonary infection rate of patients with thoracic spinal cord injury and paraplegia.METHODS A total of 108 patients with thoracic spinal cord injury and paraplegia in our hospital from Feb.2015 to Jan.2017 were selected as the research subjects.The patients were randomly divided into control group and observation group using the digital random table method,with 54 cases in each group.The control group was treated with conventional nursing measures to prevent patients with pulmonary infections,and the observation group was treated with enhanced pulmonary function exercise based on the routine nursing measures.The incidence of pulmonary infections,pulmonary function index of oxygen partial pressure(PaO2),oxygenation index(PaO2/FiO2),maximal ventilation(MVV),forced expiratory volume in one second(FEV1),respiratory dysfunction rate,and respiratory function grading evaluation between the two groups were compared.RESULTS Three months after the group,the PaO2,PaO2/FiO2,MVV and FEV1 increased in the two groups(P〈0.05),and PaO2,PaO2/FiO2,MVV,and FEV1 in the observation group were significantly higher than those in the control group(P〈0.05).The pulmonary infection rate of the patients in the observation group was 3.70% within 3 months,which was lower than 12.96% of the control group,but there was no significantly difference between the groups(P=0.060).The rate of respiratory dysfunction was12.96%,which was lower than 24.07% of the control group(P=0.031).The respiratory function classification of the two groups was significantly decreased after 3 months,and the number of 0~ Ⅱ cases in the observation group was more than that of the control group,but there was no significant difference between the groups(P=0.161).CONCLUSIONThe enhanced pulmonary function exercise can effectively im
