摘要
目的观察负压封闭引流技术(Vacuum sealing drainage,VSD)对单双侧锚钉治疗胫腓骨骨折患者术后感染的效果。方法选择2010年1月-2017年6月医院骨外科收治的98例行单双侧锚钉治疗的胫腓骨骨折术后感染患者,将患者按随机数表法分为试验组50例和对照组48例,试验组采用VSD治疗,对照组采用传统局部换药治疗,观察比较两组患者治疗效果及生活质量等情况。结果试验组患者术后感染控制时间(12.1±1.4)d、骨折愈合时间(7.9±0.8)月、清洁创面时间(22.1±4.6)d、清理创面后闭合时间(8.3±1.5)d、换药次数为(4.3±0.8)次,均短于对照组患者(P<0.05);治疗后3d,试验组患者血清CRP水平、体温及疼痛评分均低于对照组患者(P<0.05),试验组患者治疗效果及生活质量的改善均优于对照组患者(P<0.05)。结论对单双侧锚钉治疗胫腓骨骨折术后感染患者采用VSD治疗临床效果佳,能有效控制患者术后感染并缩短感染时间,促进创面和骨折部位愈合,该方法安全性高,患者疼痛感减轻,生活质量有所提高。
OBJECTIVE To observe the effect of vacuum sealing drainage（VSD）on postoperative infections of patients with tibial and fibular fractures treated with single and double lateral anchors.METHODS A total of 98 patients with tibial and fibular fractures treated by single and double lateral anchors in department of orthopeadic surgery were selected from Jan.2010 to Jun.2017,and were randomly divided into observation group（n=50）and control group（n=48）by random number table.The observation group was treated with VSD,and the control group was treated with traditional local dressing.The therapeutic effect and quality of life were compared between the two groups.RESULTS In the observation group of patients,the control time of infection was（12.1±1.4）d,healing time was（7.9±0.8）months,wound cleaning time was（22.1±4.6）d,closure time after wound cleaning was（8.3±1.5）d,and times of dressing was（4.3±0.8）,which were all shorter than those in the control group.Three days after treatment,the serum CRP level,body temperature and pain score in patients of the observation group were significantly lower than those of the control group（P〈0.05）,and the effective rate and the improvement of quality of life of the observation group were significantly better than those of the control group（P〈0.05）.CONCLUSIONIt is effective to treat patients with postoperative infection,shorten the infection time and promote the healing of the wound and fracture site with tibial and fibular fractures treated with single and double lateral.The method is safe,the pain of patients is alleviated,and the quality of life of patients is improved.
