摘要
目的:研究吲哚氰绿(ICG)排泄试验与肝脏硬度值、Child-pugh评分之间的关系,探讨乙肝肝硬化患者的肝脏储备功能与肝脏硬度值之间的关系。方法:分析我院收治的67例乙肝肝硬化患者临床资料,将患者分为乙肝肝硬化代偿期及乙肝肝硬化失代偿期两组,对两组患者进行ICG排泄试验,监测ICG K和ICG R15;利用实时剪切波弹性成像进行肝硬度值定量检测,同时计算患者Child-pugh评分。结果:随着肝脏硬度值的增加,ICG K值降低,ICG R15值增加,两组间比较,差异有显著统计学意义(P<0.01)。随着肝脏硬度值的增加,患者Child-pugh评分也增高,两组间比较差异有显著统计学意义(P<0.01)。随着Child-pugh评分的增高,ICG R15值升高,ICG K值下降,差异有统计学意义(P<0.01)。结论:乙肝肝硬化患者的肝脏储备功能随着患者肝脏硬度值的增加而降低,对肝脏硬度值的检测有助于衡量慢性肝病患者肝功能贮备的情况。吲哚菁绿实验联合实时剪切波弹性成像可准确了解患者肝脏受损的严重程度,准确评估肝脏储备功能。
Objective：The aim of this paper is to study the relationship between indocyanine green（ICG）excretion tests and liver stiffness values and Child-pugh scores and explore the relationship between liver reserve function and liver stiffness in patients with hepatitis B cirrhosis.Method：The clinical data of 67 patients with hepatitis B cirrhosis admitted to our hospital were analyzed.The patients were divided into two groups：hepatitis B liver cirrhosis compensatory phase and hepatitis B liver cirrhosis decompensated phase.ICG excretion tests were performed on two groups of patients to monitor ICG K and ICG R15.The realtime shearing wave elastography was used to quantify the hepatic stiffness,while calculating the Childpugh score.Results：With the increase of liver stiffness,ICG K values decreased and ICG R15 values increased with a statistically significant difference between the two groups（P〈0.01）.The higher the liver hardness increases,the more the patient＇s Child-pugh score increases,in which there was significant difference between the two groups（P〈0.01）.With the increase of Child-pugh score,ICG R15 value increased,while ICG K value decreased,the difference between which was statistically significant（P〈0.01）.Conclusions：The hepatic reserve function of patients with hepatitis B cirrhosis decreases with the increase of liver stiffness,and the detection of hepatic hardness can help to measure the liver function reserve in patients with chronic liver disease.Indocyanine green experiment combined with real-time shearing wave elastography can not only accurately understand the severity of liver damage in patients but also accurately assess liver reserve function.
