摘要
目的探讨腹腔镜结直肠癌手术与开放手术的远期疗效。方法选取60例结直肠癌患者,随机分为2组,观察组(30例)给予腹腔镜手术治疗,对照组(30例)给予开腹手术治疗。根据手术情况、术后病理情况、随访3年期间生存情况,评价腹腔镜结直肠癌手术与开放手术的远期疗效。结果观察组住院时间短于对照组、手术时间长于对照组、术中出血量少于对照组(P<0.05);观察组切除淋巴结总数为(12.56±4.36)枚,多于对照组(10.41±3.45)枚(P<0.05)。2组在转移淋巴结数及肿瘤分化程度方面相比,无统计学差异(P>0.05)。观察组1年、3年生存率均高于对照组,但无统计学差异(P>0.05);总复发率上比较,2组亦无统计学差异(P>0.05)。结论腹腔镜手术与开腹手术对结直肠癌的治疗效果相当,但腹腔镜手术创伤小,患者恢复快,并且能适度改善患者预后,提高生存率,是理想的手术治疗方式。
Objective To explore the long-term effect of laparoscopic colorectal cancer operation and open operation.Methods 60 patients with colorectal cancer were randomly divided into the control group and the observation group. The observation group（ 30 cases） received laparoscopic surgery,the control group（ 30 cases） received laparotomy. Evaluating the longterm efficacy of laparoscopic colorectal cancer surgery and open surgery by operation,postoperative pathology,follow-up 3 years of survival. Results The length of hospital stay and peroperative bleeding of the observation group was shorter than that of the control group（ P〈0. 05）. The operation time of the observation group was longer than that of the control group（ P〈0. 05）. The total number of resected lymph nodes in the observation group（ 12. 56 ± 4. 36） was more than that of the control group（ 10. 41 ±3. 45）（ P〈0. 05）. There was no significant difference between the 2 groups in the number of metastatic lymph nodes and the degree of tumor differentiation（ P〉0. 05）. The 1-year and 3-year survival rates of the observation group were higher than those of the control group,but there was no significant difference（ P〉0. 05）. There was no significant difference between the 2 groups in total relapse rate（ P〉0. 05）. Conclusion Laparoscopic surgery of colorectal cancer has same therapeutic effect as open operation. The surgical trauma of laparoscopic surgery is small and patients recover quickly. Laparoscopic surgery is the ideal surgical treatment,can moderately improve the prognosis and survival rate of patients.
