社区老年人群鼾症发病情况及与骨质疏松症的关系研究 被引量:3

Incidence of sleep apnea syndrome in community-dwelling elderly population and its relationship with osteoporosis

摘要 目的 探讨社区老年人群鼾症发病情况及与骨质疏松症的关系.方法 选取本社区110例阻塞性睡眠呼吸暂停(OSAHS)患者,根据呼吸暂停低通气指数(AHI)将OSAHS患者分为:轻度OSAHS组(n=39)、中度OSAHS组(n=45)、重度OSAHS组(n=26),另选正常受试者45例作为对照组,观察比较各组受试者血液生化指标(Ca2+、p3+、ALP、β-CTX)及各组腰椎骨、股骨和胫骨密度(BMD)测量数值,同时AHI与各指标进行Pearson相关性分析.结果 各组间BMI指数和SaO2min指标差异均具有统计学意义(P<0.05),中度和重度OSAHS组患者BMI指数显著升高(P<0.05),而SaO2min指标显著低于对照组及轻度OSAHS组(P<0.05).各组研究对象血液中Ca2+、P3+、ALP指标均无显著性差异(P>0.05),与对照组相比,各组OSAHS组患者β-CTX显著升高,随着患者病情程度加重,β-CTX含量显著升高(P<0.05).中度和重度OSAHS组患者腰椎骨、股骨和胫骨BMD较对照组及轻度OSAHS患者显著降低(P<0.05),且重度OSAHS患者较中度OSAHS患者腰椎骨、股骨和胫骨BMD显著降低(P<0.05).经Pearson相关性分析显示,BMI指数、β-CTX与AHI呈正相关(r=0.334、0.384,P<0.05),而SaO2min与AHI呈负相关(r=-0.801,P<0.05),Ca2+、P3+、ALP、腰椎骨、股骨颈、全髋BMD与AHI无显著相关性.结论 本社区OSAHS发病与老年人骨折疏松密切相关,其中低氧血症可能是发生骨质疏松的危险因素. Objective To investigate the incidence of sleep apnea syndrome in the elderly population and its relationship with osteoporosis.Methods A total of 110 patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) in the community were included in the study.According to the apnea hypopnea index (AHI),all patients were divided into the mild OSAHS group (n=39),moderate OSAHS group (n=45) and severe OSAHS group (n=26).Another 45 normal subjects were included as the control group.The blood biochemical parameters (Ca2+,p3+,ALP,β-CTX) and the bone mineral density (BMD) of lumbar spine,femur and tibia in all groups were determined and compared.Pearson correlation analysis was performed for AHI and the parameters.Results There were statistically significant differences in BMI and SaO2min among the groups (P〈0.05).The BMI in the moderate and severe OSAHS groups significantly increased (P〈0.05),while the SaO2min was significantly lower than that in the control group and mild OSAHS group (P〈0.05).There were no significant differences in the blood Ca2+,p3+ and ALP among the groups (P〉 0.05).Compared with the control group,the levels of β-CTX in the OSAHS groups significantly increased,and the content of β-CTX increased significantly with the aggravation of the disease (P〈0.05).The BMD of lumbar spine,femur and tibia in the moderate and severe OSAHS groups was significantly lower than that in the control group and mild OSAHS group (P〈0.05).The BMD of lumbar spine,femur and tibia significantly decreased in the severe OSAHS group compared with that in the moderate OSAHS groups (P〈0.05).The Pearson correlation analysis showed that BMI and β-CTX were positively correlated with AHI (r=0.334,0.384,P〈0.05),and SaO2min was negatively correlated with AHI (r=-0.801,P〈0.05).There was no significant correlation between Ca2 +,p3 +,ALP,BMD of lumbar spine,femur and tibia and AHI.Conclusion The incidence of OSAHS in this community is closely r
