摘要
目的:用Meta分析评价乳腺癌易感基因(BRCA)突变对前列腺癌易感性的影响。方法:通过计算机检索Pub Med、Embase、Cochrane图书馆、Web of Science、Springer Link等数据库,对纳入的研究运用Revman 5.3软件以及stata 14.0软件进行Meta分析。结果:共纳入23篇文献,共47 726例患者,其中BRCA1基因突变人群5 745例,BRCA2基因突变人群5 127例。Meta分析结果显示:BRCA1基因对前列腺癌易感性(OR:1.62,95%CI:0.83~3.15,P=0.16)、Gleason评分(OR:1.39,9 5%CI:0.8 9~2.1 5,P=0.1 5)、T分期(OR:0.84,95%CI:0.44~1.58,P=0.59)、总生存时间(OR:1.12,95%CI:0.64~1.95,P=0.69)无统计学差异;在BRCA2基因对前列腺癌易感性(OR:2.24,95%CI:1.70~3.44,P<0.000 01)、Gleason评分(OR:4.34,95%CI:3.06~6.15,P<0.000 01)、T分期(OR:2.33,95%CI:1.09~4.96,P=0.03)、总生存时间(OR:1.53,95%CI:1.37~1.71,P<0.000 01)有显著性差异。结论:BRCA2基因突变的人群患前列腺癌的风险以及预后明显比非突变人群的要高。
Objective: Meta analysis was used to evaluate the effect of breast cancer susceptibility gene( BRCA)mutation on prostate cancer susceptibility.Methods: We searched the Pub Med,Embase,Cochrane library,Web of Science,Springer,were searched to get related studies.Data analysis was conducted by Revman 5.3 software and stata 14.0 s of tware.Results: Twenty-three clinical controlled trials with 4 7 7 2 6 cases were included in this analysis,5 745 were BRCA1 mutation and the 5 127 were BRCA2 mutation.No statistically significant differences of the susceptibility( OR: 1.62,95% CI: 0.83 ~ 3.15,P = 0.16),Gleason score( OR: 1.39,95% CI: 0.89 ~ 2.15,P = 0.15),T-stage( OR: 0.84,95% CI: 0.44 ~ 1.58,P = 0.59),overall survival( OR: 1.12,95% CI: 0.64 ~ 1.95,P = 0.69) in BRCA1 mutation.There was significant differences of susceptibility( OR: 2.24,95% CI: 1.70 ~ 3.44,P 0.000 01),Gleason Score( OR: 4.34,95% CI: 3.06 ~ 6.15,P 0.000 01),T-stage( OR : 2.3 3,9 5 % CI : 1.09 ~ 4.96,P =0.03),overall survival( OR: 1.53,95% CI: 1.37 ~ 1.71,P 0.000 01) in BRCA2 mutation.Conclusion: Mutation of BRCA2 had a great influence of prognosis and susceptibility of patients with prostate cancer.
