摘要
目的探讨老年2型糖尿病(T2DM)患者血清高迁移率族蛋白B1(HMGB1)水平勺认知功能的相关性。方法选取老年T2DM患者75例,根据简易智能精神状态检查量表(MMSE)评分将其分为认知正常组和认知障碍组;另选取同期老年健康体检者43例作为对照组。采用酶联免疫吸附试验(ELISA)检测3组研究对象的HMGB1水平,并采用重复性成套心理状态测验(RBANS)对其进行认知功能评定。结果与对照组比较,认知正常组与认知障碍组血清HMGB1水平升高,且认知障碍组较认知正常组升高明显(P〈0.05)。与对照组比较,认知正常组及认知障碍组在RBANS标准化测验总分、即刻记忆、言语功能、注意力和延时记忆方面的得分均明显下降,且认知障碍组明显低于认知正常组(P〈0.05)。相关性分析结果显示,老年T2DM患者血清HMGB1水平与RBANS标准化测验总分、即刻记忆、言语功能、注意力和延时记忆均呈负相关(P〈0.05);校正性别、年龄及受教育年限后行多元逐步回归分析结果显示,血清HMGB1水平与RBANS标准化测验总分、即刻记忆、注意力、延迟记忆均呈负相关(P〈0.05)。结论老年q2DM患者的认知功能与HMGB1水平具有相关性,HMGB1水平升高可能参与了认知功能损伤的发病过程。
Objective To investigate the association of serum high mobility group box-1 protein (HMGB1) levels with cognitive function in elderly patients with type 2 diabetes mellitus (T2DM). Methods 75 cases of elderly patients with T2DM were collected and divided into the normal cognitive function group and the cognitive impairment group according to their mini-mental state examination (MMSE) scores. Forty-three cases of elderly healthy in health checks were selected as the control group in the same period. HMGB1 level was detected by enzyme-linked immuno sorbent assay ( ELISA ) and cognitive function by repeatable battery for the assessment of neuropsychologieal status (RBANS) in three groups. Results Compared with the control group, serum HMGB1 level in the normal cognitive function group and the cognitive impairment group were significantly elevated, and was significantly higher in the cognitive impairment group than that in normal cognitive function group ( P 〈 0.05 ). Compared with the control group, the standardized tests scores of RBANS,immediate memory,language function, attention and delayed memory areas in the normal cognitive function group and the cognitive impairment group were significantly decreased, and were significantly lower in cognitive impairment group compared with the normal cognitive function group(P 〈0.05 ). Correlation analysis showed serum HMGB1 levels in patients with T2DM were negatively correlated with the standardized tests scores of RBANS, immediate memory, language function ,attention and delayed memory( P 〈 0.05 ). Multivariate stepwise regression analysis was performed after adjusting for gender, age and years of education. The results showed that the level of serum HMGB1 was negatively correlated with the standardized tests scores of RBANS, immediate memory, attention and delayed memory (P 〈 0.05). Conclusion The cognitive function of elderly patients with T2DM was related to serum HMGB1 level. The increased serum HMGB1 levels may be involved
