摘要
目的对比分析胸部X线与CT在周围型肺癌检查中的征象改变特点,进一步评估X线与CT在疾病诊断中的临床价值,以提高周围型肺癌的影像学诊断水平。方法选取2014年8月—2015年10月在该院就诊且经手术、病理综合确诊的77例周围型肺癌患者,所有患者均行胸部X线正侧位摄片与CT增强扫描,对比两种检查方法的基本征象,将检查结果与手术、病理综合确诊结果比较计算诊断准确率。结果 X线与CT检查方法在病灶边缘改变[分叶征(58.44%vs 80.52%,χ~2=7.402)、毛刺征(7.79%vs 40.26%,χ~2=9.156)]、病灶内部改变[空泡征(3.90%vs 31.17%,χ~2=13.879)、空气支气管征(7.79%vs 41.56%,χ~2=6.054)]、病灶周围改变[血管集束征(32.47%vs75.32%,χ~2=10.244)、胸膜凹陷征(0.00%vs 10.39%,χ~2=8.237)]各种征象上比较,差异有统计学意义(P<0.05)。CT检查的诊断准确率为93.51%,明显高于对照组的80.52%,两种方法的诊断准确率比较,差异有统计学意义(χ~2=8.604,P<0.05)。结论 CT在周围性肺癌中的诊断准确率明显高于X线检查,将两者结合起来综合分析影像学特征具有重要的临床意义,建议在周围性肺癌临床诊治中推广应用。
Objective To compare the characteristics of chest X-ray and CT scans in the detection of peripheral lung cancer and further evaluate the clinical value of X-ray and CT in the diagnosis of the disease to improve the imaging diagnosis of peripheral lung cancer. Methods A total of 77 patients with peripheral lung cancer who were diagnosed in the hospital from August 2014 to October 2015 were diagnosed by surgery and pathology. All patients underwent chest X-ray radiographs and contrast-enhanced CT scans. The basic signs of the examination methods are to compare the examination results with the comprehensive diagnosis results of surgery and pathology to calculate the diagnostic accuracy. Results X-ray and CT examinations were performed at the lesion edge [leaf sign(58.44% vs 80.52%, χ^2=7.402),burr sign(7.79% vs 40.26%, χ^2=9.156)], lesion internal change [empty] Focal sign(3.90% vs 31.17%, χ^2=13.879), air bronchogram(7.79% vs 41.56%, χ^2=6.054)], lesion circumference change [Bundle sign(32.47% vs 75.32%, χ^2=10.244),pleural recession sign(0.00% vs 10.39%, χ^2=8.237)] a variety of signs, the difference was statistically significant(P〈0.05). The diagnostic accuracy of CT examination was 93.51%, which was significantly higher than the 80.52% of the control group. The diagnostic accuracy of the two methods was statistically significant(χ^2=8.604, P〈0.05). Conclusion The diagnostic accuracy of CT in peripheral lung cancer is significantly higher than that of X-ray examination. It is of great clinical significance to combine these two methods to comprehensively analyze the imaging features. It is recommended that CT be widely used in the diagnosis and treatment of peripheral lung cancer.
