摘要
目的对长石质烤瓷贴面的效果进行长期跟踪,评价效果并分析影响长期疗效的因素。方法前瞻性入选沈阳军区总医院浑南医院口腔科2009年1月至2011年6月收治的68例(413颗)前牙、前磨牙长石质烤瓷贴面修复患者作为研究对象,定期门诊随访至2017年12月,并参照改良美国公共健康协会评价标准进行疗效评价。收集患者修复体可能的影响因素,包括咬合接触情况、不良习惯、覆颌情况、菌斑指数、瓷粉种类、基牙着色程度、牙体预备类型、修复体颈缘位置、使用遮色瓷、使用遮色粘接树脂等,采用Logistic回归分析对长期疗效的影响因素进行分析。结果最终63例(391颗)完成1.8~7.6年的随访及疗效评价。6、12、24、36、48、60、72、84、90个月长石质烤瓷贴面修复前牙、前磨牙存留率分别为100.0%(391/391)、98.5%(385/391)、94.1%(352/374)、93.5%(316/338)、90.0%(224/249)、81.2%(160/197)、82.1%(101/123)、75.6%(62/82)、76.0%(38/50)。Logistic回归分析显示,不良习惯、使用不当、正中颌接触或前伸颌接触、重度四环素牙或重度氟斑牙或重着色牙是影响修复效果的危险因素(比值比分别为5.802、4.804、2.915、3.002,P<0.05)。结论长石质烤瓷贴面长期效果良好,使用不当、不良习惯、咀嚼时正中颌接触或前伸颌接触及重度四环素牙或重度氟斑牙或重着色牙可导致修复失败。
Objective To conduct long-term follow-up on the effect of long stone ceramic veneer,and evaluate the effect and analyze the factors that affect long-term efficacy. Methods A retrospective study was performed on 68 cases of patients with 413 anterior teeth,premolar feldspathic porcelain veneers who were admitted from January 2009 to June 2011. Patients were performed regularly every 6 months until December 2017,and the therapeutic effect was evaluated with reference to the improved American public health association evaluation criteria. Patients restoration possible influencing factors were collected,including occlusal contact situation,bad habits,jaw,plaque index,porcelain powder type,degree of abutment coloring,the types of tooth preparation,restoration of the neck edge location,used color porcelain,used color adhesive resin on the assignment,using Logistic regression analysis to analyze the influence factors of long-term efficacy. Results The final 63 cases（ 391 pieces） were followed up for 1. 8-7. 6 years and the efficacy was evaluated. After 6,12,24,36,48,60,72,84 and 90 months,feldspathic porcelain cover repair of anterior teeth,premolar retention were100. 0%（ 391/391）,98. 5%（ 385/391）,94. 1%（ 352/374）,93. 5%（ 316/338）,90. 0%（ 224/249）,81. 2%（ 160/197）,82. 1%（ 101/123）,75. 6%（ 62/82） and 76. 0%（ 38/50）,respectively. Logistic regression analysis showed that the center of the bad habits,improper use,jaw contact or forward jaw,severe tetracycline teeth or severe dental fluorosis or recolor tooth were influence repair effect of risk factors（ OR = 5. 802,4. 804,2. 915,3. 002,P〈0. 05）. Conclusion Long-term effect of feldspathic porcelain cover is good,use undeserved,bad habits,in the middle of the jaw contact or forward jaw contact as chewing and severe tetracycline teeth or severe dental fluorosis or recolor teeth can lead to the failure repair.
