八项主要血细胞分析指标的测量不确定度评估及分析 被引量:2

摘要 目的:根据ISO15189实验室认可要求,评估8项主要血常规指标的测量不确定度.方法:采用室内质控获得的中间精密度计算血常规指标测量重复性引入的测量不确定度,采用卫生部室间质量评价结果计算血常规指标偏倚引入的测量不确定度,然后计算出合成标准不确定度和扩展不确定度.结果:8项主要血常规指标的相对扩展不确定度(95%置信限)如下:WBC的相对测量不确定度为2.68%~5.44%;RBC的相对测量不确定度为2.40%~3.66%;Hb的相对测量不确定度为2.08%~2.62%;HCT的相对测量不确定度为3.66%~4.08%;PLT的相对测量不确定度为7.74%~8.94%.MCV的相对测量不确定度为2.42%~4.04%;MCH的相对测量不确定度为2.94%~3.42%;MCHC的相对测量不确定度为3.44%~4.10%.结论:8项主要血液常规指标的测量不确定度评估结果可为临床实验室改进质量提供依据,并用于解释临床结果. Objective: To evaluate the measurement uncertainty of eight kinds of main blood routine indexes according to laboratory accreditation requirements as to ISO 15189. Methods: The uncertainty of measuring repeatability of eight kinds of main blood routine indexes were obtained from intermediate precision by testing the internal quality control. The bias uncertainty was obtained from testing results of external quality assessment of National Center for Clinical laboratories. And then the combined standard uncertainty and expanded uncertainty were calculated. Results: Related expanded uncertainties of the eight kinds of main blood routine indexes with 95% confidence limit were as follows: WBC (2.68%-5.44%) , RBC (2.40%-3.66%) , Hb (2.08%-2.62%) , HCT (3.66%-4.08%) , PLT (7.74%-8.94%) , MCV (2.42% -4.04%) , MCH (2.94%-3.42%) , MCHC (3.44%-4.10%) . Conclusion: The evaluation results of measurement uncertainty can provide the basis for improving the quality of clinical laboratory, which is benefit for explaining the clinical results.
