阻抗法与多普勒超声心动图法测量新生儿心输出量一致性研究 被引量:2

Study on the Consistency between Impedance Technique and Echocardiogram Cardiac Output Measurements

摘要 【摘要】目的探讨阻抗法测量新生儿,输出量的准确性。方法对30例新生儿同时使用无创血流动力学监测系统(NICaS)和多普勒超声心动图(ECHO)测量心输出量(CO),采用Bland—Ahman法比较2种方法的一致性。结果30例新生儿平均孕周为(37.5±1.9)周,出生体重为(2913+423)g,男性比例为56.7%(17/30)。ECHO测量的每搏输出量(ECHO—SV)与NICaS测量的sV(NICaS.SV)分别为(4.45+0.78)mL和(4.54+0.81)mL,差值平均值一0.09mL,一致性区间(LoA)为(一1.92,1.73)mL,矫正后的精准度为:27.3%。ECHO测量的心输出量(ECH—O—CO)与NICaS测量的CO(NICaS—CO)分别为(0.62_+0.12)L/min和(0.61±O.12)L/rain,差值平均值0.01L/min,LoA为(一033,0.31)L/rain,矫正后的精准度为:28.3%。结论阻抗法与ECHO测量心输出量一致性良好,但还需要通过更多病例数来进一步验证。 Objective To explore the accuracy and precision of impedance technique cardiac out- put measurement. Methods A total of 30 neonates with 30 pairwise cardiac output (CO) measurements of impedance technique and echocardiogram (ECHO) were performed. Data analysis was used to study the con- sistency between impedance technique from NICaS and ECHO cardiac output measurements, and results are presented in Bland-Ahman plot. Results The mean gestational age were (37.5±1.9) weeks, mean birth weight were (2913±423) g, and seventeen neonates were male (17/30). The stroke volume (SV) by ECHO (ECHO-SV) and by NICaS (NICaS-SV) were (4.45±0.78) mL and (4. 54±0. 81) mL, respec- tively. The bias and limits of agreement (LoA) of NICaS-SV were 0.09 mL, ( -1.92, 1.73) mL, respec- tively. The true precision of NICaS-SV is 27.3%. The cardiac output (CO) by ECHO (ECHO-CO) and by NICaS (NICaS-CO) were (0.62+0.12) L/min and (0.61±0.12) L/min, respectively. The bias and LoA of NICaS-CO were 0.01L/min and ( -0.33, 0.31) L/min, respectively. The true precision of NICaS- CO is 28.3%. Conclusions Agreement between the impedance technique and ECHO in the cardiac output measurement is acceptable. Because of lack of a large number of patients, the accuracy and precision need to be further estimated.
