摘要
目的:探讨腹腔镜手术对卵巢囊肿患者术后应激及卵巢储备功能的影响。方法:将某院收治的108例卵巢囊肿患者作为观察对象,依照随机数表法将其分为2组,每组各54例。对照组行经腹卵巢囊肿剥除术,观察组实施腹腔镜卵巢囊肿剥除术。观察2组手术情况、治疗前后卵巢储备功能、应激指标变化情况。结果:2组患者术中出血量、手术时间、下床时间比较,对照组高于观察组,差异有统计学意义(P<0.05);治疗前2组去甲肾上腺素(NE)、超敏C-反应蛋白(hs-CRP)、血清皮质醇(Cor)、卵巢最大直径、窦状卵泡数、抗苗勒管激素(AMH)、抑制素B(INHB)水平对比,差异无统计学意义(P>0.05);治疗后对照组NE,hs-CRP,Cor,AMH水平高于观察组,卵巢最大直径、窦状卵泡数、INHB水平低于观察组,差异有统计学意义(P<0.05)。结论:将腹腔镜手术用于卵巢囊肿的治疗中有助于加快卵巢储备功能的恢复,减轻机体应激程度,促进患者早日康复。
Objective： To observe the effect of laparoscopic surgery on postoperative stress response and ovarian reserve function in patients with ovarian cyst.Methods： 108 patients with ovarian cyst were selected and randomly divided into two groups,with 54 cases in each group.The control group underwent transabdominal ovarian cystectomy,while the observation group was treated by laparoscopic ovarian cystectomy. The surgical conditions,ovarian reserve function,changes of stress indexes of the two groups were observed. Results： The intraoperative bleeding volume,operation time,leaving bed time in the control group were higher or longer than those in the observation group（ P〈0.05）.Before treatment,there was no statistical difference in terms of the levels of norepinephrine（ NE）,highsensitivity C-reactive protein（ hs-CRP）,serum cortisol（ Cor）,ovarian maximum diameter,antral follicle count,anti-mullerian hormone（ AMH）,and inhibin B（ INHB） between the two groups（ P〈0.05）. After treatment,the levels of NE,hs-CRP,Cor,AMH in the control group were higher than those in the observation group,while the ovarian maximum diameter,antral follicle count,and INHB level were lower than those in the observation group（ P〈0.05）.Conclusion： Laparoscopic surgery in treatment of ovarian cyst can accelerate restoration of ovarian reserve function,reduce stress response and promote patients＇ early recovery.
