显微CT在上颌第一前磨牙根管形态中的研究价值 被引量:4

摘要 目的探讨人上颌第一前磨牙根尖孔解剖形态的增龄性变化,为临床对不同年龄患者的上颌第一前磨牙进行根管治疗提供理论指导和依据。方法收集拔除的30颗有年龄标记的上颌第一前磨牙进行标记,分成三组(A组:20~39岁,B组:40~59岁,C组:≥60岁),每组10颗,采用SCANCO MEDICALμ-CT 80显微CT断层扫描,MIMICS 8.0分析软件测量根尖孔的直径,利用SPSS 13.0统计软件对数据进行统计分析。结果上颌第一前磨牙生理性根尖孔直径平均值为(0.253±0.075)mm,A与C组间差异有统计学意义(P<0.05),B与C组差异有统计学意义(P<0.05),2组间差异无统计学意义(P>0.05);上颌第一前磨牙生理性根尖孔形状圆形占22.45%,椭圆形占61.22%,不规则形占16.33%,3组根尖孔形状的差异无统计学意义(P>0.05);上颌第一前磨牙以单根牙和双根牙多见,三根牙较少见,单根牙和双根牙在三组间发生率差异无统计学意义(P>0.05);根管类型中单根管(Ⅰ型)发生率为33.33%,双根管(Ⅱ~Ⅶ型)发生率为63.33%,三根管(Ⅷ型)发生率为3.33%。结论上颌第一磨牙根管系统解剖形态复杂,上颌第一前磨牙根尖孔直径在老年组减少,而根尖孔形状无增龄性变化,主要为椭圆形,临床以单根管型和双根管型最为多见。 Objective To investigate the changes of anatomic form of apical foramen of maxillary first premolar in order to provide theoretical basis forroot canal therapy of maxillary first premolar in different age patients. Methods A total of30 exelcymosis maxillary first premolars with age marker were divided into three groups: group A( 20 to 39 years),group B( 40 to 59 years),group C( 60 years and above),with 10 premolars in each group. The SCANCO MEDICAL μ-CT 80 micro CT tomoscan was used to examined these premolars,and MIMICS 8. 0 analysis software was use to measur the diameter of the root hole,moreover SPSS 13. 0 statistical software was used to analyze the results. Results The physiological root hole diameter of maxillary first premolar was average( 0. 253 ± 0. 075) mm,there was significant difference between group A and group C( P〈 0. 05) and between group B and group C( P〈 0. 05). However there was no significant difference between group A and group B( P 〉0. 05). The physiological root hole shape of maxillary first premolar was round( 22. 45%),ellipse( 61. 22%) and irregular shape( 16. 33%),but there was no significant difference in apical foramen shape among the three groups( P 〉0. 05). The single rooted tooth and double rooted tooth were commonly found in maxillary first premolar,and triple rooted tooth was relatively rare,and there was no significant difference in the incidence rate of single rooted tooth and double rooted tooth among the three groups( P〉 0. 05). In root canal type,the incidence rate of single tube( typeⅠ) was 33. 33%,that of double root canal( Ⅱ ~ Ⅶ) was 63. 33%,and that of three root canal( Ⅷ) was 3. 33%. Conclusion The maxillary first molar root canal system anatomical shape is complex,the apical foramen diameter of maxillary first premolar is decreased in the elderly,while the apical foramen shape is not changed with age increasing,which is mainly elliptic,however simple root canal and double root canal are m
