摘要
目的探讨重症颅脑损伤机械通气患者中留置鼻肠管行肠内营养的应用效果。方法研究资料均为本院于2014年12月~2016年12月期间收治的重症颅脑损伤机械通气患者,共58例。按照随机数字抽取表法进行分组,对照组采用鼻胃管肠内营养支持方案,共29例,观察组采用留置鼻肠管行肠内营养支持方案,共29例,对两组患者的营养指标改善情况、恢复情况及不良反应发生情况进行比较分析。结果治疗后两组患者各指标均改善,其中观察组血清白蛋白、血红蛋白、呼出潮气量、氧合指数明显高于对照组,而血尿素氮明显低于对照组,差异有统计学意义(P<0.05);观察组机械通气时间与ICU住院时间均显著低于对照组,差异有统计学意义(P<0.05);观察组反流、误吸、腹泻、应激性溃疡等不良反应发生率(17.24%),低于对照组不良反应发生率(71.37%),差异有统计学意义(P<0.05)。结论在重症颅脑损伤机械通气患者治疗行留置鼻肠管行肠内营养支持方案利于改善患者营养水平,并控制不良反应的发生,缩短机械通气时间和住院时间,临床应用价值较高。
Objective To investigate the effect of enteral nutrition with indwelling nasal tube in patients with severe craniocerebral injury. Methods All the data were collected from 58 patients with severe traumatic brain injury who were treated in our hospital from December 2014 to December 2016. Are grouped according to the random number table extraction method, the control group using nasogastric tube enteral nutrition, a total of 29 cases, observation group were treated with indwelling nasal tube enteral nutrition support program, a total of 29 cases, recovery and adverse reaction incidence were compared and analyzed, on the nutritional index of the two groups of patients. Results After treatment, the indexes of two groups of patients were improved in the observation group, serum albumin, hemoglobin, expiratory tidal volume and oxygenation index was significantly higher than the control group, the blood urea nitrogen was significantly lower than the control group, P 〈 0.05; the observation group duration of mechanical ventilation and ICU stay were significantly lower than the control group, P 〈 0.05 observation group; reflux, aspiration, diarrhea, stress ulcer and other adverse reaction rate of 17.24%, lower than the control group adverse reaction rate was 71.37%, P 〈 0.05. Conclusion Indwelling nasal tube for enteral nutrition can improve the nutritional level of patients in the treatment of patients with mechanical ventilation in patients with severe craniocerebral injury, and to control the occurrence of adverse reactions, shorten the time of mechanical ventilation and hospitalization time, the higher the value of clinical application.
