不同脂肪分层对肥胖2型糖尿病抵抗素、内脂素水平影响 被引量:3

Influenc of Different Fat Layers on Resistin and Alfatin Levels in Obese Patients With Type 2 Diabetes

摘要 目的观察肥胖程度不同的2型糖尿病患者抵抗素及内脂素水平并分析其影响因素。方法按腰围分为腰围正常组65例(A组)、腰围轻度增高组64例(B组)、腰围中度增高组68例(C组)、腰围重度增高组68例(D组)。比较各组BMI、FPG、2 h PG、Hb A1C、TG、TC、HMOA-IR、血清抵抗素、内脂素水平,并分析抵抗素和内脂素水平的影响因素。结果随着腰围的增加,抵抗素水平有增加趋势,但各组间差异无统计学意义(P>0.05);内脂素:随着腰围的增加,内脂素水平有所增加,各组间差异有统计学意义(P<0.05);相关分析,各组抵抗素水平与TC均相关(P<0.05),与其他观察指标未显示相关性(P>0.05)。各组内脂素与WC、BMI、FPG、2 h PG、HOMA-IR、Hb A1c、TG、TC具有相关关系(P<0.05或P<0.05),结论抵抗素与中心性肥胖程度无相关性,抵抗素可能影响伴有肥胖的2型糖尿病患者的脂质代谢;2型糖尿病患者内脂素水平与中心性肥胖程度有关,内脂素可能参与2型糖尿病和肥胖的发病机制。 Objective To observe the levels of resistin and alfatin in type 2 diabetic patients with different levels of obesity and to analyze the influencing factors. Methods According to waist circumference, they were divided into different groups: normal waist group(65 cases, group A), waist circumference mild increase group(64 cases, group B), waist circumferencemoderate increase group(68 cases, group C), waist circumference severe increase group 68 cases(group D). The levels of BMI, FPG, 2 h PG,HbA1 C, TG, TC, HMOA-IR, serum resistin and alfatin were compared in each group, and the factors affecting the levels of resistin and alfatin were analyzed. Rusults With the increase of waist circumference, the level of resistin increased, but there was no statistical difference between each group(P〉0.05); Alfatin: with the increase of waist circumference, the level of endogenous lipoprotein increased, and there was a statistical difference between each group(P〈0.05). The correlation analysis showed that the levels of resistin in each group were correlated with TC(P〈0.05), and had no correlation with other observational indexes(P〉0.05). The relationship among WC, BMI, FPG, 2 h PG, HOMA-IR, HbA1 c, TG and TC in each group was related(P〈0.05 or P〈0.05). Conclusion Resistin is not related to the degree of central obesity, and resistin may affect lipid metabolism in patients with type 2 diabetes with obesity. The level of alfatin in patients with type 2 diabetes is associated with the degree of central obesity. Alfatin may be involved in the pathogenesis of type 2 diabetes and obesity.
