摘要
目的探讨胃肠外科腹腔镜技术在基层医院复制的学习曲线。方法选取2014年9月—2016年9月来该院行腹腔镜胃手术患者37例,另选取同时间段来该院行腹腔镜结直肠手术患者37例,收集所有患者的临床资料并进行回顾性分析,对两组患者治疗有效率及治疗满意度进行对比分析。结果所有患者均成功完成腹腔镜手术,胃癌患者手术时间为3~4 h,结直肠癌患者手术时间为2~4 h,所有患者在围手术期未出现严重的并发症。术后有1例患者因结直肠癌死于胃瘫。胃肠手术组治疗有效率为70.3%,结直肠手术组治疗有效率为78.4%,差异有统计学意义(P<0.05)。另外,胃部手术组护理满意度为89.2%,结直肠组护理满意度为91.8%,差异有统计学意义(P<0.05)。结论在开腹手术基础上,对解剖技术熟练掌握,并借鉴他人成功的腹腔镜胃肠手术经验及手术组医务人员之间的默契合作是基层医院开展腹腔镜胃肠手术学习曲线"复制"的关键。
Objective This paper tries to explore the learning curve of gastrointestinal surgery laparoseopic technique in primry hospital. Methods From September 2014 to September 2016, 37 patients underwent laparoscopic gastrecto- my in this hospital were selected, 37 patients undergoing laparoscopic colorectal surgery at this hospital for the same period of time were also selected. Clinical data of all patients were collected and reviewed. Treatment efficiency and treatment satisfaction of the two groups of patients were compared and analyzed. Results Laparoscopie surgery was suc- cessfully performed in all patients. The operation time of gastric cancer patients was 3-4 hours, and the operation time of colorectal cancer patients was 2-4 hours. All patients did not have serious complications during the perioperative period. One patient died of gastric fistula due to eoloreetal cancer. The effective rate was 70.3% in the gastrointestinal surgery group and 78.4% in the colorectal surgery group. The difference was statistically significant （P〈0.05）. In addi- tion, the satisfaction degree of the nursing in the stomach surgety group was 89.2%, and the satisfaction degree of the colorectal group was 91.8%. The difference was statistically significant （P〈0.05）. Conclusion On the basis of laparoto- my, mastery of anatomy techniques, and learning from the successful experience of laparoscopic gastroenterology and the tacit cooperation between medical personnel in the surgery group are the keys to the ＂replication＂ of laparoscopic gastrointestinal surgery learning curve in basic hospitals.
