摘要
目的观察针刺联合补阳还五汤治疗难治性面瘫的临床效果与安全性。方法选择2013年1月~2016年12月上海中医药大学附属曙光医院收治的难治性面瘫患者70例,按照随机数字表法将其分为治疗组(35例)和对照组(35例)。对照组给予针刺治疗,治疗组在对照组的基础上联合补阳还五汤治疗,两组患者均连续治疗1个月。记录两组患者的中医症状评分、面神经功能分级量表评分和面部残疾指数量表评分,观察两组患者治疗后总有效率以及不良反应事件发生率。结果治疗1个月后,治疗组临床总有效率为88.57%,高于对照组的74.29%,差异有统计学意义(P<0.05)。两组患者治疗后中医症状评分均较本组治疗前下降,治疗组治疗1个月后中医症状评分低于对照组(P<0.05)。两组患者治疗后面神经功能分级均较治疗前改善,治疗组治疗1个月后面神经功能分级优于对照组(P<0.05)。两组患者治疗后面部残疾指数躯体功能评分、社会生活功能评分均较治疗前升高,治疗组治疗1个月后面部残疾指数躯体功能评分、社会生活功能评分高于对照组(P<0.05)。治疗组与对照组治疗后不良反应发生率比较,差异无统计学意义(P>0.05)。结论针刺联合补阳还五汤能够明显改善难治性面瘫患者中医症状、面神经功能、面部残疾指数,其确切的疗效与良好的安全性为该治疗方案的广泛推广奠定了坚实的基础。
Objective To observe the clinical efficacy and safety of acupuncture combined with Buyang Huanwu Decoction in the treatment of refractory facial paralysis. Methods Seventy patients treated for refractory facial paralysis in Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from January 2013 to December2016 were selected, and divided into treatment group（35 cases） and control group（35 cases） by random number table method. The control group was treated with acupuncture therapy, while the treatment group was treated with Buyang Huanwu Decoction based on the treatment of control group, all the patients were treated for 1 month. The TCM syndrome score, facial nerve function rating scale and facial disability index score were recorded, the total effective rate and incidence of adverse events were observed. Results After 1 month of treatment, the total effective rate of the treatment group was 88.57%, which was higher than that of the control group（74.29%）（P〈0.05）. After treatment, the TCM symptom scores of the two groups were lower than those before treatment, and the score of treatment group after treatment was lower than that of the control group（P〈0.05）. The neurological function classification of the two groups after treatment was improved compared with those before treatment. After treatment, the neurological function classification of the treatment group was better than that of the control group（P〈0.05）. Facial disability index score and physical function social of the two groups were higher than those before treatment, and the score of the treatment group was higher than the control group（P〈0.05）. There was no statistically significant difference of adverse events between treatment group and control group（P〉0.05）. Conclusion Acupuncture combined with Buyang Huanwu Decoction can significantly improve symptoms, facial nerve function and facial disability index of refractory facial paralysis patients.Exact efficacy and good safety of the tr
