摘要
目的探讨负压吸引装置联合输尿管镜钬激光碎石治疗输尿管上段结石的临床疗效。方法回顾性分析本单位2014年10月至2016年8月间采用输尿管镜钬激光碎石术治疗的117例输尿管上段结石患者临床资料。所有患者均为单侧输尿管结石,其中对照组(在术中单独采用输尿管镜钬激光碎石术)70例,观察组(在术中同时应用负压吸引装置配合碎石)47例,比较两组的结石上移率、手术成功率、术后1个月结石清除率、手术时间及术后发热的发生情况。结果术中对照组和观察组结石上移率分别为31.43%(21/70)和6.38%(3/47)(P<0.05)。对照组和观察组的碎石成功率分别为74.29%(52/70)和93.62%(44/47)(P<0.05)。对照组与观察组手术时间分别为(69±24)分钟和(63±20)分钟(P>0.05)。术后对照组有12例患者出现发热,观察组有2例患者出现发热(P<0.05)。术后1个月对照组和观察组手术成功患者结石清除率分别为81.43%(57/70)和95.74%(45/47)(P<0.05)。结论负压吸引装置联合输尿管钬激光碎石术在治疗输尿管上段结石时可以显著降低结石上移率、提高手术成功率及结石清除率,是一种安全、有效的方法。
Objective To investigate the clinical effect of negative pressure suction device combined with ureteroscopic holmium laser lithotripsy for the treatment of upper ureteral calculi. Methods The clinical data of117 cases of upper ureteral calculi treated with ureteroscopic holmium laser lithotripsy from October 2014 to August 2016 were analyzed retrospectively. All the patients were diagnosed as unilateral ureteral calculi. 70 cases underwent ureteroscopic holmium laser lithotripsy as the control group,47 cases underwent ureteroscopic holmium laser lithotripsy combined with negative pressure suction device as the observation group. The stone removal rate,the success rate,the stone clearance rate,the operation time and the occurrence of postoperative fever of the two groups were compared. Results In the control group and the observation group,the lithiasis shift rate was31. 43%（ 22/70） and 6. 38%（ 3/47）,respectively（ P〈0. 05）. The success rates of lithotripsy in the control group and the observation group were 74. 29%（ 52/70） and 93. 62%（ 44/47）,respectively（ P〈0. 05）. The operative time in the control group and the observation group were（ 69 ± 24） minutes and（ 63 ± 20） minutes,respectively,but there was no significant difference between these two groups（ P〈0. 05）. 12 patients had fever after operation in the control group,while 2 patients had fever in the observation group（ P〈0. 05）. One month after the operation,the stone clearance rate in the control group and the observation group was 81. 43%（ 57/70） and 95. 74%（ 45/47）,respectively（ P〈0. 05）. Conclusions The treatment for upper ureteral calculi with ureteroscopic holmium laser lithotripsy combined with negative pressure suction device can reduce the rate of lithiasis shift significantly,improve the success rate of operation and the rate of stone clearance. It is a safe and effective method.
