血清CRP、降钙素原及血清、腹水淀粉酶对重症急性胰腺炎的预判价值 被引量:2

摘要 目的探讨血清C反应蛋白(CRP)、降钙素原(PCT)、腹水淀粉酶在重症急性胰腺炎中检测预判价值。方法选取2017年3月—2018年1月我院收治腹水患者40例作为研究对象,根据腹水来源分为对照组(非胰源性腹水,20例)和实验组(重症急性胰腺炎伴腹水,20例),监测两组患者24 h、48 h、72 h内CRP、PCT、血清淀粉酶和腹水淀粉酶水平,入院72h内完成腹部CT检查,进行CT严重程度指数(CTSI)评分,分析CRP、PCT、血清淀粉酶和腹水淀粉酶水平与CTSI相关性。结果对照组患者CRP、PCT、血清淀粉酶和腹水淀粉酶水平均低于实验组(P<0.05)。对照组患者CTSI分级显著低于实验组(P<0.05)。CRP、PCT、血清淀粉酶和腹水淀粉酶与CTSI分级呈均正相关性(P<0.05)。结论 CRP、PCT、血清淀粉酶和腹水淀粉酶水平与重症急性胰腺炎严重程度呈正相关性,具有重要检测和预判价值。 Objective: To investigate the predictive value of serum C-reactive protein( CRP),procalcitonin( PCT) and ascites amylase in severe acute pancreatitis. Method: Select 40 case of ascites patients in our hospital during Mar.,2017—Jan.,2018 as studying object,according to the origin of ascites,the patients were divided into control group( non-pancreatic ascites,20 cases) and experimental group( severe acute pancreatitis with ascites,20 cases),the levels of CRP,PCT,serum amylase and ascites amylase were monitored within 24 hours,48 hours and 72 hours in both groups. Abdominal CT examination was performed within 72 hours after admission,and the CT severity index( CTSI) was scored. The levels of CRP,PCT,ascites amylase levels and CTSI correlation. Result: The levels of CRP,PCT,serum amylase and ascites amylase in the control group were lower than those in the experimental group( P〈0. 05). The CTSI score of the control group was significantly lower than that of the experimental group( P〈0. 05). CRP,PCT,serum amylase and ascites amylase were positively correlated with CTSI grade( P〈0. 05). Conclusion: CRP,PCT,serum amylase and ascites amylase levels and the severity of severe acute pancreatitis was positively correlated,with important detection and predictive value.
