摘要
目的探讨多层螺旋CT、超声及X射线钼靶扫描对乳腺癌的诊断作用。方法我院124例乳腺癌患者,术前均接受CT、超声和X射线钼靶影像学特征,并与手术病理检测结果进行对比,分析CT、超声及X射线钼靶对乳腺癌的诊断作用。结果 124例患者术后病理检测均确诊为乳腺癌。CT扫描显示均为肿块型,多呈不规则、圆形和椭圆形病变,病灶边缘模糊或清晰,多呈分叶状,多为边缘毛刺,肿块密度高于正常组织,浸润性24.19%,伴钙化54.84%,胸壁肌肉受侵10.48%,淋巴结肿大62.90%。超声检测显示多为不规则团块,多实质性弱回声,内部强回声,多呈分叶状,部分回声减弱,无包膜,呈浸润型生长,多发生淋巴结转移,部分血库伯韧带受侵,血流絮状。X射线钼靶扫描多为不规则团块,多伴钙化,边缘模糊,分叶状,毛刺征,部分伴外钙化,皮肤改变,结构絮乱,少部分高密度影,晕环,多发生淋巴结转移。阳性检出率CT为87.10%,超声为80.65%,X射线钼靶为83.87%,三者联合检测为96.77%,联合检出率显著高于单用检出率(P<0.05)。结论 CT、超声及X射线钼靶均对乳腺癌具有较高诊断价值,三者联合检测阳性检出率更高,利于乳腺癌及早发生和治疗。
Objective To compare the diagnostic values of multi-slice spiral CT,ultrasonography and X-ray mammography in breast cancer.Methods One hundred and twenty-four patients with breast cancer in our hospital underwent examination of CT,ultrasound and X-ray mammography before surgery.The results were compared with the surgical pathology.The diagnostic values of CT,ultrasound and X-ray mammography were analyzed.Results All 124 patients were confirmed as breast cancer after postoperative pathological examination.CT scan showed mass type mainly with irregular,round and oval lesions,and blurred or clear lesions edges with mostly lobulated shapes and burrs on edges.The masses had higher density than normal tissue.There were 24. 19% invasions,54. 84% calcifications,10. 48% chest wall muscle invasion and 62. 90% lymph node enlargement. Ultrasonography showed mostly irregular mass with multiple weak echo and internal strong echo.The images were mostly lobulated shapes with partial weakened echo.The mass had no capsule and infiltrative growth.The lymph node metastasis occurred with part of the blood Kuberan ligament invasion and flocculent blood flow.X-ray mammography scanning showed irregular masses with calcification,blurred edge,lobulation and speculation. Part of masses was external calcification,skin changes and structure disorders.A small part was high density images,halo and high occurrence rate of lymph node metastasis.The positive detection rate in CT was 87. 10%,ultrasound 80. 65%,and X-ray mammography 83. 87%,respectively.However,the rate could be 96. 77% when the three image examinations were combined and was significantly higher than any one lone( P〈0. 05).Conclusion CT,ultrasonography and X-ray mammography have higher diagnostic values for breast cancer,and the combined three ways can create a higher positive rate which is beneficial to early diagnosis and treatment of breast cancer.
