摘要
目的:探究艾司西酞普兰与奥氮平联用治疗老年抑郁症效果及对患者认知和神经内分泌功能的影响。方法:2014年1月~2016年12月我科住院120例老年抑郁症患者,随机分成联用组与单用组各60例,单用组服用艾司西酞普兰,联用组增服奥氮平,比较两组疗效、认知功能及神经内分泌指标的变化。结果:治疗3个月后,两组HAMD评分均显著低于治疗前(P<0.05),联用组显著低于单用组(P<0.05);两组MMSE评分均显著高于治疗前(P<0.05),联用组显著高于单用组(P<0.05);联用组与单用组血清S100B、MBP浓度均较治疗前明显下降(P<0.05),联用组明显低于单用组(P<0.05);两组治疗前后TSH变化不明显(P>0.05)。结论:艾司西酞普兰与奥氮平联合治疗老年抑郁症效果显著,可以提升患者认知功能,改善其血清S100B、MBP水平。
Objective： To explore the efficacy of escitalopram combined with olanzapine on elderly patients with depression and its effects on cognitive function and neuroendocrine function. Methods： 120 cases of elderly patients with depression hospitalized in our department from January 2014 to December 2016 were selected for the study and randomly divided into combined group（ n = 60） and single group（ n = 60）. The single group was given escitalopram,and the combined group was additionally given olanzapine. The efficacy,cognitive function and effects of drugs on neuroendocrine function were compared between the two groups. Results： After 3 months of treatment,the HAMD score of two groups was significantly lower than that before treatment（ P〈0. 05）,and the score in combined group was significantly lower than that in single group（ P〈0. 05）. The MMSE score of two groups was significantly higher than that before treatment（ P〈0. 05）,and the score in combined group was significantly higher than that in single group（ P〈0. 05）. The concentrations of S100 B and MBP in two groups were significantly lower than those before treatment（ P〈0. 05）,and the concentrations in combined group were significantly lower than those in single group（ P〈0. 05）,and the change of TSH in the two groups was not obvious before and after treatment（ P〉0. 05）. Conclusions： Escitalopram combined with olanzapine has significant effects in the treatment of elderly patients with depression,and it can improve cognitive function,and enhance serum levels of TSH,S100 B and MBP.
