摘要
目的对血液灌流救治急性百草枯中毒的临床疗效与可行性进行系统评价,为其临床应用提供参考依据。方法方便选取2016年6月—2018年2月该院急诊收治的20例百草枯中毒患者,分为观察组与对照组各10例,均行洗胃、导泻、利尿等常规治疗,观察组在此基础上进行血液灌流治疗,比较两组治疗效果。结果观察组救治成功率为70.0%,存活率为90.0%,均明显高于对照组(χ~2=9.403、7.651,P<0.05);观察组MOF(10.0%vs 40.0%,χ~2=7.241)与ARDS(30.0%vs 50.0%,χ~2=8.165)发生率均明显低于对照组(P<0.05);观察组血液灌流时出现1例血压下降、1例穿刺部位出血,经相应治疗后好转未影响血液灌流正常进行。结论血液灌流是治疗急性百草枯中毒的有效手段,可明显提高救治成功率与存活率,而且并发症少,安全性高,值得临床推广。
Objective This paper tries to evaluate the clinical efficacy and feasibility of hemoperfusion in treating acute paraquat poisoning and provide a reference for its clinical application. Methods 20 cases of paraquat poisoning patients admitted to this hospital from June 2016 to February 2018 were convenient selected and divided into observation group and control group each with 10 cases. Routine treatment such as gastric lavage, catharsis and diuresis were performed. The observation group added blood perfusion treatment, compare the treatment effect of the two groups. Results The treatment success rate in the observation group was 70.0%, and the survival rate was 90.0%, which was significantly higher than that of the control group（χ~2=9.403,7.651,P〈0.05）; the observation group MOF（10.0% vs 40.0%, χ~2=7.241） and ARDS（30.0% vs50.0%, χ~2=8.165）. The incidence was significantly lower than that of the control group（P〈0.05）. In the observation group,blood pressure decreased in 1 case and bleeding in the puncture site occurred in 1 case. Improvement after appropriate treatment did not affect blood perfusion and worked properly. Conclusion Perfusion of blood is an effective means for the treatment of acute paraquat poisoning. It can significantly increase the success rate and survival rate of treatment, and has fewer complications and high safety. It is worthy of clinical promotion.
