摘要
目的探讨尿激酶静脉溶栓联合丁苯酞序贯治疗急性缺血性脑卒中的临床疗效。方法收集我院96例急性缺血性脑卒中患者,随机分成实验组和对照组各48例,两组患者均予尿激酶静脉溶栓及常规治疗,实验组在此基础上联合丁苯酞序贯治疗,观察90d,比较两组患者治疗前后卒中量表(NIHSS)评分、日常生活活动能力量表(ADL)评分及临床疗效。结果 (1)治疗后14d、90d,两组患者NIHSS评分均低于本组治疗前,且实验组显著低于对照组,差异具有统计学意义(P<0.05)。(2)治疗后14d、90d,两组患者ADL评分均高于本组治疗前,且实验组显著高于对照组,差异具有统计学意义(P<0.05)。(3)实验组总有效率为91.7%,对照组总有效率为75.0%,实验组总有效率显著高于对照组,差异具有统计学意义(P<0.05)。结论尿激酶静脉溶栓联合丁苯酞治疗急性缺血性脑卒中疗效优于单纯应用尿激酶静脉溶栓,有利于患者神经功能康复,提高日常生活活动能力,值得临床推荐应用。
Objective To investigate the clinical efficacy of urokinase intravenous thrombolysis combined with butyphthalide in sequential treatment of acute ischemic stroke. Methods Ninety-six patients with acute ischemic stroke were randomly divided into experimental group and control group, 48 cases in each group. Both groups were applied to urokinase intravenous thrombolysis and conventional treatments, but the experimental group was also given butyphthalide in sequential treatment. After 90 days observation, the NIHSS scores, the ADL scores and the clinical efficacy of both groups were compared before and after treatment. Results（1）14 days and 90 days later, the NIHSS scores in both groups were significantly lower than those before treatment, but the score of the experimental group was obviously less than that of the control group（P〈0.05）.（2）14 days and 90 days later, the ADL scores in both groups were higher than those before treatment, but the score of the experimental group was obviously more than that of the control group（P〈0.05）.（3）The total efficacy of the experimental group was 91.7%, while that of the control group was 75%, so the experimental group was superior to the control group（P〈0.05）. Conclusion Urokinase intravenous thrombolysis combined with butylphthalide is superior to urokinase intravenous thrombolysis alone in the treatment of acute ischemic stroke, which is conducive to the rehabilitation of neurological function and improve the ability of daily living. It is worthy of clinical recommendation.
