摘要
目的探讨心率变异性(HRV)在不同危险分层的急性冠脉综合征(ACS)中的预测价值。方法选取我院收治的100例ACS患者为研究对象,所有患者于入院24 h内给予24 h连续动态心电图监测。根据ACS患者入院时GRACE评分将患者分为非高危组(≤133分,n=49)与高危组(>133分,n=51)。分析两组HRV各指标水平;根据24 h动态心电图监测结果,比较两组心律失常发生情况。结果高危组SDNN、SDANN、SDNNI、rMSSD、PNN50及HF水平明显低于非高危组,高危组LF/HF水平明显高于非高危组(P<0.05);高危组心律失常发生率明显高于非高危组(P<0.05)。结论 HRV各指标可有效预测高危ACS,以便临床医师尽早开通病变血管,减少心血管不良事件发生。
Objective To investigate the predictive value of heart rate variability（HRV） analysis applied in high risk acute coronary syndrome（ACS）.Methods A total of 100 cases of patients with ACS were selected as the study objects. All the patients performed 24 h continuous electrocardiogram monitoring within 24 h after admission. The patients were divided into non-high-risk group（GRACE score≤133, n=49） and high-risk group（GRACE score133, n=51） according to the GRACE scores of the patients at admission. The levels of HRV indexes of the two groups were analyzed; and the incidence of arrhythmia was compared between the two groups according to the results of 24 h continuous electrocardiogram monitoring.Results The levels of SDNN, SDANN, SDNNI, rMSSD, PNN50 and HF in the high-risk group were lower than those in the non-high-risk group, while the LF/HF level of the high-risk group was higher than that of the non-high-risk group（P〈0.05）. The incidences of arrhythmia in the high-risk group were higher than those in the non-high-risk group（P〈0.05）.Conclusion The HRV of atrial indexes can predict high-risk patients with ACS effectively, so that clinicians can open the diseased vessels as early as possible to reduce the incidence of cardiovascular adverse events.
