摘要
目的对比分析鼻内镜下鼻腔泪囊造孔术与鼻腔泪囊吻合术治疗慢性泪囊炎的临床疗效。方法便利选择该院2012年10月—2015年12月收治的78例(92眼)慢性泪囊炎患者,将其随机分为观察组(47眼,行造孔术治疗)和对照组(45眼,行吻合术治疗),对比两组围手术相关指标、临床疗效及并发症情况。结果观察组手术时间(t=8.665)、术中出血量(t=11.927)、术后鼻腔渗血量(t=5.449)及住院时间(t=6.785)均明显小于对照组(P<0.05),治愈率为82.98%,明显高于对照组(χ~2=8.734,P<0.05),两组治疗总有效率比较差异无统计学意义(χ~2=0.235,P>0.05),并发症发生率为2.56%,明显低于对照组的15.38%(χ~2=9.474,P<0.05)。结论鼻内镜下鼻腔泪囊造孔术治疗慢性泪囊炎是一种安全性高、创伤小的术式,有利于患者术后恢复,值得临床推广。
Objective To compare and analyze the clinical curative effect of endoscopic pore nasal lacrimal surgery and dacryocystorhinostomy in treatment of chronic dacryocystitis. Methods 78 cases of patients with chronic dacryocystitis（92 eyes） admitted and treated in our hospital from October 2012 to December 2015 were selected and randomly divided into two groups, the observation group with 47 eyes were treated with dacryocystorhinostomy, while the control group with 45 eyes were treated with anastomosis, and the related indicators, clinical curative effect and complications were compared between the two groups. Results The operation time, intraoperative bleeding amount, postoperative nasal osmotic volume and length of stay in the observation group were obviously lower than those in the control group（t=8.665,11.927,5.449,6.785, P0.05）, and the cure rate was 82.98%, which was obviously higher than that in the control group（χ~2=8.734, P 0.05）, and the difference in the total effective rate between the two groups was not obvious（χ~2=0.235, P0.05）, and the incidence rate of complications was obviously lower than that in the control group（2.56% vs 15.38%）,（χ~2=9.474, P0.05）. Conclusion The endoscopic pore nasal lacrimal surgery in treatment of chronic dacryocystitis is a operation way with high safety and small wound, which is conductive to the recovery after surgery, and it is worth clinical promotion.
