摘要
目的探讨布地奈德福莫特罗粉、噻托溴铵联合无创通气应用于慢性阻塞性肺疾病(COPD)伴Ⅱ型呼吸衰竭中的效果。方法方便选择2014年8月—2016年8月该院收治的COPD伴Ⅱ型呼吸衰竭患者98例,以随机数字表法将其分为对照组(行常规对症治疗措施)与研究组(在对照组的基础上应用布地奈德福莫特罗粉、噻托溴铵联合无创通气治疗),每组49例。观察对比两组临床治疗效果、治疗前后肺功能与血气指标的变化。结果研究组治疗的总有效率为95.92%,高于对照组79.59%(χ~2=6.078,P<0.05)。治疗后研究组FEV1(2.82±0.52)L、FEV1占预计值百分比(75.60±4.20)%、FEV1/FVC(78.85±4.65)均高于对照组(1.98±0.22)L、(64.56±4.52)%、(66.58±4.58)(t=3.856,4.526,4.985,P<0.05),PaO_2(77.85±6.65)mmHg高于对照组(70.50±5.68)mmHg(t=4.252,P<0.05),PaCO_2(60.50±6.58)mmHg低于对照组(68.88±5.60)mmHg(t=4.356,P<0.05)。结论布地奈德福莫特罗粉、噻托溴铵联合无创通气应用于COPD伴Ⅱ型呼吸衰竭患者中效果满意,可以有效改善其肺功能与血气指标,值得临床推广。
Objective To study the effect of budesonide formoterol powder, tiotropium bromide combined with noninvasive ventilation in COPD with type Ⅱ respiration failure. Methods 98 cases of patients with COPD with type Ⅱ respiration failure admitted and treated in our hospital from August 2014 to August 2016 were conveniently selected and randomly divided into two groups, with 49 cases in each, the control group used the routine systematical treatment measures, and the research group used the budesonide formoterol powder, tiotropium bromide combined with noninvasive ventilation on the basis of the control group, and the clinical treatment effect, and changes of lung function and blood gas indicators were compared between the two groups. Results The total treatment effective rate in the research group was higher than that in the control group（95.92% vs 79.59%）（χ~2=6.078, P0.05）, after treatment, the FEV1, percentage of FEV1 in the predicative values and FEV1/FVC in the research group were higher than those in the control group[（2.82 ±0.52）L,（75.60 ±4.20） %,（78.85 ±4.65）vs（1.98±0.22）L,（64.56±4.52）%,（66.58±4.58）（t=3.856, 4.526, 4.985, P0.05）, and the PaO2, was higher than those in the control group [（77.85±6.65）mmHg,（70.50±5.68）mmHg（t=4.252, P0.05） ], and the PaCO2 was lower than that in the control group [（60.50±6.58）mmHg vs（68.88±5.60）mmHg（t=4.356, P0.05） ]. Conclusion The application effect of budesonide formoterol powder, tiotropium bromide combined with noninvasive ventilation in COPD with type Ⅱ respiration failure is satisfactory, which can effectively improve the lung functions and blood gas index, and it is worth clinical promotion.
