摘要
目的分析甲状腺良恶性结节钙化超声的特点。方法对2015年1—10月于北京大学第一医院行甲状腺彩超检查并行甲状腺结节切除术,术后经石蜡切片病理检查的755个甲状腺结节进行回顾性分析,并将术前甲状腺超声钙化征象(微小钙化、粗大钙化、混合钙化、无钙化)与术后的病理结果进行对比分析。结果 755个甲状腺结节中良性331个,恶性424个(包括乳头样癌405个、髓样癌8个、滤泡样癌4个、低分化癌2个、可疑恶性5个)。恶性结节患者的年龄小于良性结节患者[(45.1±11.6)岁比(51.4±12.0)岁](P<0.01)。有钙化的甲状腺结节良恶性比例高于无钙化患者,微小钙化的恶性结节比例高于粗大钙化(P<0.05)。结论超声钙化对甲状腺结节的良恶性诊断及鉴别有明显特异性,具有一定的临床应用价值。
Objective To analyze and discuss the ultrasonic calcific features of benign and malignant thyroid nodules.Methods A total of 755 thyroid nodules obtained after thyroid color Doppler ultrasonography and thyroid nodule resection with postoperative pathological examination of paraffin section in the First Hospital of Beijing University from January to October 2015 were retrospectively analyzed, and preoperative ultrasound calcification signs （minor calcification,coarse calcification ,mixed calcification,and no calcification） were compared with the postoperative pathological results. Results Among the 755 thyroid nodules,there were 331 benign and 424 malignancies（including 405 papillary carcinomas,8 medullary carcinomas,4 follicular carcinomas,2 poorly differentiated carcinomas,and 5 suspicious malignant lesions ）.The patient age of malignant nodules was lower than that of benign nodules [（45.1±11.6） year old vs （51.4± 12.0 ） year old]（ P 〈0.01）. The ratio of benign and malignant thyroid nodules with calcification was higher than that without calcification,and the proportion of malignant nodules with microcalcification was higher than that with coarse calcification（ P 〈 0.05）. Conclusion Ultrasonographic calcific features have obvious specificity and clinical value in the diagnosis and identification of the benign and malignant thyroid nodules.
