2013年—2016年某综合医院5种重要及特殊耐药菌的检出及科室分布情况分析 被引量:1

摘要 目的分析我院2013年—2016年5种重要及特殊耐药菌的检出率及科室分布情况。方法采用纸片扩散法或珠海DL-96Ⅱ全自动细菌鉴定药敏仪统一按全国细菌耐药监测方案进行药物敏感性试验,按CLSI 2014年版标准判断结果。结果耐甲氧西林金黄色葡萄球菌、耐碳青霉烯类的铜绿假单胞菌整体表现呈下降趋势,检出率分别为23.6%~40.8%和19.2%~29.6%;对三代头孢耐药的大肠埃希菌、肺炎克雷伯菌和耐碳青霉烯类鲍曼不动杆菌检出呈上升趋势,检出率分别为49.4%~74.45%、26.4%~42.3%和21.3%~63.0%。5种重要及特殊耐药菌的科室分布主要在普外科(16.3%)、ICU(15.29%)、神经内科(11.54%)、呼吸内科(9.52%)和神经外科(7.97%)。结论 5种重要及特殊耐药菌科室分布表现出一定的聚集性,检出率呈波浪变化,其中对三代头孢耐药的大肠埃希菌、肺炎克雷伯菌和耐碳青霉烯类鲍曼不动杆菌呈上升趋势。上述结果表明细菌耐药监测对有效治疗细菌感染和医院感染控制十分重要。 Objective To analyze the detection rate and distribution of 5 important and special resistant bacteria in our hospital from 2013 to 2016.Methods The drug sensitivity test was carried out by the Zhuhai DL-96Ⅱautomatic bacterial identification drug sensitivity apparatus according to the national surveillance program of bacterial resistance.According to CLSI 2014 edition standard judgment result. Results Of methicillin resistant Staphylococcus aureus and the pseudomonas aeruginosa of carbapenems-resistant overall was decreased, the detection rate was 23.6%~40.8% and19.2%~29.6%; the results showed that the three generations of ceft-resistant escherichia coli,klebsiella pneumoniae and carbapene-resistant Acinetobacter baumannii overall was increased,the detection rate was 49.4%~74.45%, 26.4%~42.3% and 21.3% ~63.0%. The 5 important and special drug-resistant bacteria were mainly distributed in department of general surgery(16.3%), ICU(15.29%), Neurology(11.54%),Respiratory medicine(9.52%) and Department of Neurosurgery(7.97%). Conclusion The distribution of 5 important and special drug-resistant bacteria families was clustered. The detection rate was wavy. Among them, the detection rate of three generation cephalosporin resistant Escherichia coli, Klebsiella pneumoniae and carbapene-resistant Bauman Acinetobacter was increasing. These results show that bacterial resistance monitoring is very important for effective treatment of bacterial infection and control of hospital infection.
