摘要
目的:评价术前预防性输注头孢呋辛钠或头孢硫脒对妇科手术患者心电图心室复极指标的影响。方法:择期妇科手术患者50例、美国麻醉医师协会(ASA)分级Ⅰ或Ⅱ级,随机均分为头孢呋辛钠组(F组)和头孢硫脒组(L组),于手术前分别输注相应抗生素;于抗生素输注前和输注后记录两组患者的12导联心电图,测量并比较两组患者的QT间期、心率校正QT间期(QTc间期)、Tp-e间期、Tp-e/QT比值、平均动脉压(MAP)和心率(HR)。结果:与抗生素输注前比较,抗生素输注后F组QTc和Tp-e间期延长(P<0.05),L组Tp-e间期延长、Tp-e/QT比值增大(P<0.05);与F组比较,抗生素输注后L组QT间期和QTc间期缩短(P<0.05);两组手术患者输注抗生素前后的MAP及HR比较,差异无统计学意义(P>0.05)。结论:术前预防性输注头孢呋辛钠对妇科手术患者心室复极的干扰较头孢硫脒小。
Objective： To evaluate the effect of preoperative infusion of cefuroxime sodium or cefathiamidine on ventricular repolarization in patients of gynecological operation. Methods： 50 patients with elective gynecological operation and ASA grade Ⅰ or Ⅱ were randomly divided into the cefuroxime sodium group（ group F） and the Cefathiamidine group（ group L）,and the corresponding antibiotics were given separately before the operation. 12-lead electrocardiogram of two groups was recorded before and after antibiotic infusion. QT interval,heart rate corrected QT interval（ QTc interval）,Tp-e interval Tp-e/QT ratio,mean arterial pressure（ MAP） and heart rate（ HR） were measured and compared between the two groups. Results： QTc and Tp-e intervals were prolonged in group F after antibiotic infusion,compared with those before antibiotic infusion（ P〈0. 05）. Tp-e interval in L group was prolonged,and Tp-e/QT ratio increased（ P〈0. 05）. Compared with group F,QT interval and QTc interval were shortened in group L after antibiotic infusion（ P〈0. 05）. There was no significant difference in map and HR between the two groups before and after antibiotic infusion（ P〈0. 05）. Conclusion：Interference of preoperative prophylactic infusion of cefuroxime sodium on ventricular repolarization in patients of gynecological operation is less than cefathiamidine.
