摘要
目的:分析肝内、肝外胆道残余结石患者采用胆道镜进行诊断与治疗的作用。方法:对100例肝内外胆道残余结石患者进行观察,对所有患者先后进行T管造影、胆道镜检查,之后再二者联合进行检查。对比观察三种检查方法对肝内、肝外胆道残余结石检出率、漏诊、误诊率情况,分析胆道镜对肝内、肝外胆道外胆道残余结石的诊治作用。结果:(1)胆道镜检出肝内、肝外胆道残余结石87例,阳性率为87.00%;T管造影检出肝内、肝外胆道残余结石76例,阳性率为76.00%;胆道镜、T管造影联合检测肝内、肝外胆道残余结石95例,阳性率为95.00%;三种检查方式检出率比较,差异有统计学意义(P<0.05)。(2)对100例肝内、肝外胆道残余结石患者在纤维胆道镜引导下进行胆道镜检查、取石疗后,均成功完成,一次结石取尽率达到98%。手术时间为15~95 min,术后住院时间1~5 d。结论:胆道镜对肝内、肝外胆道残余结石检出率高,胆道镜引导下进行取石手术的取石成功率高,结石清除彻底,弥补了单独手术的缺陷,在肝内、肝外胆道残余结石的诊治中具有重要作用。
Objective To analyze the diagnosis and treatment of choledochoscope the residual stones of intrahepatic and extral bile duct.Method 100 cases of the residual stones of intrahepatic and extral bile duct were observed,the patients were examined by T angiography and choledochoscopy.In this paper,the diagnosis and treatment of the residual stones of biliary tract were analyzed by comparing the detection rate and the misdiagnosis rate.Results（1）There were 87 cases of biliary calculi in the biliary duct,who were examined by choledochoscope and 87.00% positive rate.There were 76 cases of biliary calculi in the biliary duct,who were examined by T angiography and 76.00% positive rate.There were 95 cases of biliary calculi in the biliary duct,who were examined by the combination of choledochoscope and T angiography.And 95.00% positive rate.The difference in detection rate was statistically significant（P0.05）.（2）In the case of 100 cases of biliary calculi of the liver and external biliary tract,All of them were successfully treated by the choledochoscopy.one stone removal rate reached 98%.The operation time was 15-95 min,and the postoperative hospitalization time was 1 - 5 d.Conclusion There are high detection rate and move rate for the residual stones of biliary by choledochoscope.The stone is cleared thoroughly by choledochoscope,makes up the defect of the surgery alone,in the diagnosis and treatment of intrahepatic and extral bile duct and play an important role.
