摘要
目的:分析CT和MRI诊断一氧化碳中毒性脑病的价值。方法:本研究将2015年1月~2017年4月在信宜市人民医院就诊的一氧化碳中毒患者纳入研究范围,其中2015年1月-4月收治83例,2015年12月-2016年4月收治77例,2016年11月-2017年4月收治92例,共252例。将患者临床资料进行回顾性分析,旨在明确CT和MRI对一氧化碳中毒性脑病的诊断价值。结果:本研究共纳入252例CO中毒患者,经临床资料及CT、MR影像学资料明确42(16.67%)例为CO中毒性脑病,42(100%)例患者均伴有不同程度的皮层下白质水肿和肿胀,CT及MRI检出CO中毒性脑病的检出率为100.00%,无对比差异;但MRI检查在中毒后12h内、12~24h内的中毒性脑病检出率优于CT,而且MRI检查病灶更加明确、更有特征性,对比差异有统计学意义(P<0.05)。结论:MRI对CO中毒性脑病的早期检出率显著高于CT,MRI检查病灶更加明确、更有特征性,能在早期为治疗提供高价值的影像学资料。
Objective： To analyze the value of CT and MRI in the diagnosis of carbon monoxide poisoning encephalopathy. Methods： 252 cases of patients with carbon monoxide poisoning treated in Xinyi City People＇s Hospital from January 2015 to April 2017 were enrolled in the scope of the study,including 83 cases from January 2015 to April 2015,77 cases from December 2015-to April 2016 and 92 cases from November 2016 to April 2017. The clinical data of the patients were retrospectively analyzed to clarify the diagnostic value of CT and MRI in the diagnosis of carbon monoxide poisoning encephalopathy. Results： The clinical data and CT and MR imaging data showed that there were 42（16. 67%） cases of CO poisoning encephalopathy,and 42（100%） cases of patients associated with varying degrees of brain white matter edema and swelling in the a total of 252 patients with CO poisoning,and there was no difference in the detection rate of CO poisoning encephalopathy by CT and MRI both with100%. But the detection rate of poisoning encephalopathy within 12 h or 12 - 24 h after poisoning was better in the MRI examination than that of CT examination,and the lesions of MRI examination were more definite and more characteristic（ P〈0. 05）. Conclusion： The early detection rate of CO poisoning encephalopathy by MRI is significantly higher than that by CT,and MRI lesions are more definite and more characteristic,and it can provide high-value imaging data in the early treatment.
