磁共振灌注对不同级别星形细胞瘤的诊断研究 被引量:1

摘要 目的:探讨磁共振灌注成像对不同级别星形细胞瘤的诊断价值。方法:收集2012年3月至2017年12月经手术后病理诊断的星形细胞瘤32例,依据WHO(2016)中枢神经系统肿瘤组织学分类,将1~2级星形细胞瘤归为低级别组,共15例,将3~4级星形细胞瘤归为高级别组,共17例,回顾性分析低级别组与高级别组星形细胞瘤在磁共振灌注成像中各项参数对比的差异性,肿瘤内与肿瘤周围水肿的各项参数对比的差异性。结果:低级别组、高级别组肿瘤实质的rCBV比值为4.35±4.94、7.68±3.49,低级别组、高级别组肿瘤实质的rCBF比值为3.69±4.17、8.43±4.85,两组肿瘤实质的rCBV比值、rCBF比值比较差异有统计学意义。低级别组、高级别组肿瘤实质的MTT比值、TTP比值和水肿区的rCBV比值、rCBF比值、MTT比值、TTP比值比较差异均无统计学意义。结论:不同级别的星形细胞瘤在磁共振灌注成像中的表现有一定的特异性,磁共振灌注成像可以作为星形细胞瘤术前定级别的一种有效方法。 Objective: To investigate the diagnostic value of magnetic resonance perfusion imaging in different grades of astrocytoma. Methods: 32 cases of astrocytoma proved by pathology postoperatively from March 2012 to December 2017 were collected. According to the WHO(2016) histological classification of central nervous system tumors,15 cases of grade 1 ~ 2 astrocytoma were classified as low-grade group,and 17 cases of 3 ~ 4 astrocytoma were classified as highgrade group. The difference among the various parameters in magnetic resonance perfusion imaging of both low-grade group and high-grade group was analyzed retrospectively,including the difference among the various parameters of both the tumor and its peripheral edema. Results: The rCBV ratio of the tumour substantial in low-grade group and high-grade group were 4. 35 ± 4. 94 and 7. 68 ± 3. 49 respectively,and the rCBF ratio were 3. 69 ± 4. 17,8. 43 ± 4. 85 respectively.The difference of both the rCBV ratio and the rCBF ratio of the tumour substantial between the two groups was statistically significant,and the difference of both the MTT ratio and the TTP ratio of the tumour substantial between the two groups was not. There was no significant difference in MTT ratio,TTP ratio,rCBV ratio and rCBF ratio of the peripheral edema between the low-grade group and the high-grade group. Conclusion: Different grades of astrocytoma have certain specificity in magnetic resonance perfusion imaging,Magnetic resonance perfusion imaging can be used as an effective method for the preoperative grading degree of astrocytoma.
