摘要
目的探讨口服营养补充(ONS)对老年慢性阻塞性肺疾病(COPD)稳定期出院患者营养状态和肺功能的影响。方法将85例老年COPD稳定期出院患者随机分为对照组和ONS组。对照组给予常规的饮食指导,ONS组在饮食指导的基础上每日增加口服摄入450 kcal的整蛋白肠内营养制剂,持续1个月。观察两组患者出院当天和出院后90 d的NRS-2002评分、体质量指数(BMI)、血清白蛋白等营养相关指标及肺功能的变化。结果出院当天两组患者患有营养风险(NRS-2002≥3)的比例相当(P>0.05),同时两组间BMI、血清白蛋白等营养相关指标及呼吸功能亦无明显差异(P>0.05)。出院后通过口服营养补充,与对照组相比,ONS组患有营养风险的比例明显降低,BMI明显增加,血清白蛋白和前白蛋白浓度明显升高,差异具有统计学意义(P<0.05);同时,ONS组的肺功能较对照组明显改善,差异具有统计学意义(P<0.05)。结论 ONS可改善老年COPD稳定期出院患者的营养状态,增强机体的肺功能。
Objective To investigate the influence of oral nutrition supplement（ONS） on the nutritional status and lung function of elder discharged patients with stable chronic obstructive pulmonary disease（COPD）. Methods 85 elder discharged patients with stable COPD were randomized into control group and ONS group. Patients in control group received routine diet direction. Patients in ONS group received additional oral administration of whole protein enteral nutrition prepations for 450 kcal per day and lasted 1 month. The nutrition-related indicators of NRS-2002 score in the day patients were discharged and 90 days thereafter, body mass index（BMI） and serum albumin as well as the changes of lung function were observed. Results The proportions of patients with nutritional risk（NRS-2002 ≥ 3） in the day patients were discharged in two groups were similar（P〉0.05）. There was no significant difference in BMI, serum albumin and lung function between two groups（P〉0.05）. Through ONS after discharge, compared with control group, the proportion of patients with nutrition risk decreased significantly as well as the BMI and the levels of serum albumin and prealburain increased signifieantly（P〈0.05）. In the meantime, lung function in ONS group improved Statistically（P〈0.05）. Conclusion ONS could improve the nutritional status and lung function of elder discharged patients with stable COPD.
