摘要
目的系统评价针刺配合耳穴贴压治疗脑中风失眠患者的疗效。方法计算机检索PubMed、Cochrane Library、中国生物医学文献数据库(CBM)、中国期刊全文数据库(CNKI)、维普期刊资源整合服务平台(VIP)和万方数据知识服务平台(WanFang Data)等数据库中有关针刺配合耳穴贴压治疗脑中风失眠患者的随机对照试验(RCT)或半随机对照试验(CCT),追溯纳入文献的参考文献,文献检索时限为各数据库建库至2017年12月。按照纳入和排除标准筛选文献、提取资料并评价纳入研究的方法学质量后,采用RevMan 5.2软件进行Meta分析。结果共纳入7篇RCT,合计435例受试者。据Meta分析结果显示:针刺配合耳穴贴压组的总有效率高于对照组[OR=7.30,95%CI(3.51~15.18),P<0.01]。结论虽然针刺配合耳穴贴压治疗脑中风失眠的疗效优于对照组,且无毒副反应,但由于纳入研究数量和质量上存在缺陷,因此开展更多高质量、内在真实性好的大规模、多中心的RCT研究非常必要。
Objective To evaluate systematically the efficacy of acupuncture combined with auricular acupoint pressing in the treatment of insomnia patients with cerebral apoplexy.Methods PubMed,Cochrane Library,Chinese biomedical literature database（CBM）,Chinese How Net（CNKI）,VIP journal resources integration service platform（VIP） and Wan Fang database（WanFang Data） systems were searched for literature on acupuncture combined with auricular point pressing therapy in the treatment of insomnia patients with cerebral apoplexy.Literature concerning randomized controlled trials on patients with insomnia（RCT） or semi-randomized controlled trial（CCT） were collected with their publishing data from the establishement of the database to December 2017.literature mentioned in the bibliography of relevant articles were also collected.Meta analysis was carried out by RevMan 5.2 software after screening literature,extracting data and evaluating the methodological quality of the included study.Results 7 RCT were included with a total of 435 subjects included.According to the results of Meta analysis,the total effective rate of acupuncture combined with auricular acupoint pressing group was higher than that of the control group [ OR =7.30,95%CI（3.51-15.18）,P〈0. 01].Conclusion Although the effect of acupuncture combined with auricular acupoint pressing in the treatment of insomnia patients with cerebral apoplexy was better than that of the control group with no adverse reaction,there were still defects in the quantity and quality of RCT.It is necessary to carry out more high-quality,authentic,massive and multi-centered RCT researches.
