不同植骨材料在良性骨肿瘤患者植骨重建的疗效

摘要 目的探讨不同植骨材料在良性骨肿瘤患者术后植骨重建的疗效.方法分析2011 年1 月至2016 年1 月甘肃武威民勤县人民医院收治的120 例良性骨肿瘤患者,根据植骨重建术使用的植骨材料分为异体骨组,羟基磷灰石组、硫酸钙组,每组各40 例.比较3 组患者植骨后愈合速度、愈合率、经济成本.随访2~5 年,分析患者的预后情况.结果异体骨组、羟基磷灰石组、硫酸钙组术后12 个月愈合率分别为97. 5%、92. 5%、87. 5%,差异无统计学意义(P〉0. 05).异体骨组初步愈合时间短于羟基磷灰石组和硫酸钙组(P〈0. 05),羟基磷灰石组和硫酸钙组相比差异无统计学意义(P〉0. 05).术后18个月,异体骨组、羟基磷灰石组、硫酸钙组肢体功能优良率分别为92. 5%、85. 0%、82. 5%,差异无统计学意义(P〉0. 05).3 组术后KPS 评分均明显升高(P〈0. 05),异体骨组术后6 个月、12 个月KPS 评分均高于羟基磷灰石组和硫酸钙组(P〈0. 05),术后3 个月及术后18 个月3 组KPS 评分差异无统计学意义(P〉0. 05).异体骨组治疗费用0. 95~ 1. 53(1. 21±0. 57)万元;羟基磷灰石组治疗费用0. 93~ 1. 36(1. 18±0. 34)万元,硫酸钙组治疗费用0. 84 ~ 1. 17(1. 03±0. 34)万元;3 组费用比较差异无统计学意义(P〉0. 05).结论硫酸钙和羟基磷灰石人工骨植入骨愈合率与异体骨植入相似. ObjectiveTo investigate the effects of different bone graft materials on the reconstructionof bone graft in patients with benign bone tumor. MethodsFrom January 2011 to January 2016, 120 patientswith benign bone tumor were treated in our hospital, considered as the research object. According to the differ-ence of the bone graft materials, 120 cases were divided into the allogeneic bone group (n = 40), thehydroxyapatite group(n =40) and the calcium sulfate group. The healing speed, healing rate and economic costafter bone graft operation in three groups were compared. Followed-up for 2-5 years, the prognosis of threegroups was analyzed. ResultsThe healing rates of the allogeneic bone group, the hydroxyapatite group and thecalcium sulfate group were 97. 5%, 92. 5% and 87. 5%, respectively. There was no significant difference be-tween three groups (P〉0. 05). The initial healing time of the allogeneic bone group was significantly shorterthan that of the calcium sulfate group and the hydroxyapatite group (P〈0. 05),ly significant (P〉0. 05). The scores of KPS in the three groups were significantly increased (P〈0. 05). The 6-month and 12-month KPS scores of the allogeneic bone group were higher than those of the hydroxyapatite groupand the calcium sulfate group (P〈0. 05). The 3-month and 18-month postoperative KPS scores among the threegroups were not statistically significant (P〉0. 05). The total cost in the allogeneic bone group was 9,500-15,300 RMB, with an average of (12,100±5,700)RMB. The treatment cost in the calcium sulfate group was 8,400-11700 RMB (average of 10,300±3,400 RMB), and the hydroxyapatite group was 9,300~ 13,600RMB[average of (11,800 ±3,400)RMB]. There was no significant difference in the cost among the three groups(P〉0. 05). ConclusionsThe healing rates of calcium sulfate and hydroxyapatite artificial bone implants weresimilar to allogeneic bone implants.
