摘要
目的探讨256层螺旋CT在腮腺肿瘤术前诊断中的应用价值。方法回顾性分析46例经手术病理证实的腮腺肿瘤患者的CT表现,包括肿瘤的分布、形态、边界、强化程度及与周围组织关系等。结果 46例患者中,良性肿瘤42例(腺淋巴瘤25例,多形性腺瘤9例,基底细胞腺瘤5例,脂肪瘤1例,淋巴管囊肿1例,海绵状淋巴管瘤1例),恶性肿瘤4例(腮腺导管癌1例,涎腺腺癌1例,淋巴上皮癌1例,恶性多行性腺瘤1例)。42例良性肿瘤中均未发现钙化灶,4例恶性肿瘤中有2例发现钙化灶。良恶性肿瘤的区分,CT术前诊断符合率为97.8%。结论 256层螺旋CT扫描对腮腺肿瘤定位定性较准确,有助于腮腺肿瘤术前鉴别诊断及手术方案的选择。综合成像时间、辐射剂量、成本费用等指标考察,较其他影像学手段有一定优势。
Objective To investigate the diagnostic value of 256-slice CT features of parotid tumors prior to the surgery.Methods The CT manifestation of 46 cases with parotid gland tumor were retrospectively analyzed,which were confirmed surgically and pathologically（including tumor distribution,form,edge,degree of enhancement,and relationship with surrounding structure）Results All of the cases,42 cases was benign（including 25 cases of adenolymphoma,9 cases of pleomorphic adenoma,5 cases of basal cell tumor,1 cases of lymphatic cyst,1 case of lipoma）. Four cases was malignant（It was ductal carcinoma,adenocarcinoma,lymphlepithelioma and malignant multiline adenoma,respectively）. Non calcification was found in the benign tumors,however,calcification was found in a half of the 4 malignant parotid tumors. The distinguish accuracy rate of CT examination was 97.8% in parotid tumors. Conclusion 256-slice CT examination has high diagnostic value on parotid tumors.
