摘要
目的:探讨营养风险评估对重症肺炎患儿内分泌代谢和不良临床结局的影响。方法:选取2016年6月—2017年6月期间来我院治疗的重症肺炎患儿资料,将128例患儿按营养程度高、中、低分类,分别为A、B、C共3组,其中A组患儿50例、B组患儿42例、C组患儿36例。所有入选患儿月龄在1~60个月,平均月龄28±3.5月。结果:3组患儿的性别、月龄、喂养情况以及身高没有明显差异(P>0.05),而患者体重存在差异(P<0.05),统计学有意义。3组患儿在内分泌代谢比较IGF-1、脂联素、瘦素、NEFA、RBP、转甲状腺素蛋白存在显著差异(P<0.05),统计学有意义;而3组患儿的ALB、铁蛋白没有明显差异(P>0.05)。3组患儿的呼吸机辅助呼吸时间、ICU、并发症、住院时间均值对比存在显著差异(P<0.05),统计学有意义;而3组患儿的死亡、呼吸机辅助呼吸没有明显差异(P>0.05)。结论:采用营养风险评估对重症肺炎患儿内分泌代谢和不良临床结局的判断较为准确,能够有效提示医师对患儿进行必要的营养支持,从而预防患儿不良临床结局的发生率。
Objective To investigate the effects of nutritional risk assessment on endocrine metabolism and adverse clinical outcomes in children with severe pneumonia. Method A total of 128 cases of children with severe pneumonia treated in our hospital during the period of June 2016—June 2017 were selected. Totally 128 children were classified as A,B,and C in three groups according to the high,middle and low levels of nutrition,of which 50 cases in group A,42 in group B,and 36 in group C. The average age of all selected children was1 ~ 60 months,the average age of the month was 28 ± 3. 5 months. Result There was no significant difference in gender,month age,feeding status and height among the three groups( P 0. 05),but the weight of patients in the two groups was different( P 0. 05),which was statistically significant. Among the three groups there was significant difference in IGF-1 endocrine metabolism,adiponectin,leptin and NEFA,NEFA,RBP,transthyretin( P 0. 05),statistical significance. There was no significant difference among three groups of children with ALB and ferritin( P 0. 05). There was a significant difference among the three groups of respirator assisted breathing time,ICU,complication and hospital stay mean( P 0. 05),statistically significant,while there was no significant difference in death and respirator assisted respiratory among the three groups( P 0. 05). Conclusion Nutritional risk assessment is more accurate for endocrine and metabolic outcomes and adverse clinical outcomes in children with severe pneumonia. It can effectively prompt physicians to carry out necessary nutritional support for children,so as to prevent the incidence of adverse clinical outcomes.
