摘要
目的:探讨血清性激素结合蛋白(SHGB)水平对妊娠期糖尿病(GDM)孕妇妊娠结局的影响。方法:选取本院妇产科GDM孕妇148例,经血糖控制后满意120例(GDM满意组),不满意28例(GDM不满意组),另选取80例正常孕妇作为对照(正常对照组)。所有孕妇孕周在37~39周,记录并观察孕妇的妊娠结局,测定所有孕妇血清中的SHGB、FPG、FINS以及HOMA-IR指数,并分析SHGB与IR的相关性以及对妊娠结局的影响。结果:三组孕妇在不同时间段胰岛素以及C肽呈正态分布,GDM不满意组胰岛素、C肽含量均高于正常对照组和GDM满意组,正常对照组SHGB含量高于GDM满意组和GDM不满意组,差异均有统计学意义(P<0.05或0.01),SHGB水平的降低与IR的升高有一定的关联性;GDM不满意组SHGB水平低于其他两组,妊娠结局并发症的发生率明显增加。结论:SHGB水平的降低与GDM孕妇妊娠结局并发症的发生率呈正相关,其水平的降低受到IR升高的影响。
Objective：To investigate the serum sex hormone binding globulin（SHGB） level for outcome with gestational diabetes of pregnancy in pregnant women.Method：A total of 148 GDM pregnant women at obstetrics and gynecology in our hospital were selected,120 cases was satisfied after blood glucose control（GDM satisfied group）,28 cases were not satisfied（GDM dissatisfied group）,another 80 cases of normal pregnant women were selected as controls（the normal control group）.All pregnant women at 37-39 weeks gestation,recorded and watched pregnant pregnancy outcomes,then measured all patients with serum SHGB,FPG,FINS and HOMA-IR index,and analyzed SHGB correlation with IR and the impact on pregnancy outcome.Result：Three groups of pregnant women at different time periods of insulin and C-peptide showed a normal distribution,insulin,C-peptide levels of GDM dissatisfied group were higher than those of the normal control group and GDM satisfied group.The SHGB levels in the normal control group were higher than those of GDM satisfaction group and GDM dissatisfied group（P〈0.05 or 0.01）,SHGB decreased levels with IR rised had some relevance.The SHGB levels in GDM dissatisfied group were lower than those of the other two groups,the incidence of complications in pregnancy outcomes significantly increased.Conclusion：The lower level of SHGB pregnancy outcomes for women with GDM incidence of complications is positively correlated with lower levels by the IR increase.
