摘要
目的:通过分析超微血管成像(SMI)模式下子痫前期(PE)患者胎盘微循环,探讨SMI临床应用价值。方法:应用SMI模式检测孕晚期38例健康孕妇及37例PE患者的胎盘血管,计数单位面积下血管条数(n/cm^2),并检测胎盘中央部最微细动脉的血流动力学参数:搏动指数(PI)、阻力指数(RI)、收缩期最大速度与舒张末期速度之比(S/D)、时间平均流速(TAV),并与超声检查前的最高收缩压、最高舒张压、新生儿出生体重及分娩孕周进行相关性分析。结果:PE组单位面积下血管条数和TAV均低于对照组,差异均有统计学意义(Z=-3.457,P=0.001;Z=-5.199,P=0.000);PE组最微细动脉阻力指标PI、RI、S/D高于对照组,但差异均无统计学意义(P>0.05)。PE组单位面积下血管条数(n/cm^2)与最高收缩压、最高舒张压呈显著负相关(r=-0.369,P<0.01;r=-0.369,P<0.01),与新生儿出生体重及分娩孕周呈显著正相关(r=0.350,P<0.01;r=0.301,P<0.01);TAV与最高收缩压、最高舒张压呈显著负相关(r=-0.556,P<0.01;r=-0.574,P<0.01),与新生儿出生体重及分娩孕周呈显著正相关(r=0.315,P<0.01;r=0.422,P<0.01)。PI、RI、S/D值与最高收缩压、最高舒张压、新生儿出生体重及分娩孕周无显著相关性。结论:SMI模式下单位面积下血管条数及TAV反映PE胎盘微循环变化,具有一定的临床价值。
Objective: To analyze the placental microcirculation in patients with preeclampsia by superb micro-vascular imaging( SMI) mode and to explore the clinical value of SMI.Methods: Application of superb micro-vascular imaging( SMI) to detect placental vessels in 38 healthy pregnant women and 37 patients with preeclampsia,count the number of vascular strips per unit area( n/cm^2),and to detect the hemodynamic parameters of the central part of the placenta micro artery: pulsatility index( PI) and resistance index( RI),peak systolic velocity and diastolic velocity ratio( S/D),time average velocity( TAV). Correlation analysis was made between the indexes and the highest systolic pressure,the highest diastolic pressure,neonatal birth weight and gestational weeks of delivery. Results: The number of vascular strips per unit area in preeclampsia group was lower than that in control group,and the difference was significant( Z =-3.457,P = 0.001).The TAV of the preeclampsia group was lower than that of the control group,and the difference was significant( Z =-5. 199,P = 0. 000). The minimal arterial resistance indexes of PI,RI and S/D in the preeclampsia group were higher than those in the control group,while the differences were not statistically significant( P 0. 05). Vascular strips per unit area( n/cm^2) was significant negatively correlated with the highest systolic pressure and the highest diastolic pressure( r =-0. 369,P 0. 01; r =-0. 369,P 0. 01),was significant positively correlated with neonatal birth weight and gestational weeks of delivery( r = 0.350,P 0.01; r = 0. 301,P 〉0. 01).
