摘要
目的分析儿童下呼吸道感染病原学诊断中上下呼吸道吸出物的应用价值。方法选取2010年5月~2017年9月于漯河市第二人民医院收治的下呼吸道感染患儿420例为研究对象,对其BALF(肺泡灌洗液)与NPA(鼻咽抽吸物)进行收集,行多病原检测,分析BALF、NPA病原学检测结果、BALF、NPA细菌及真菌检出情况、BALF、NPA中CP、MP、病毒检出情况。结果 BALF与NPA中至少检测出一种病原阳性的患儿有386例,阳性检出率(91.9%)相较于BALF,NPA的检出率更高,差异存在统计学意义(P<0.05);BALF、NPA细菌检出总阳性患者72例(17.7%),两组检出率比较无显著差异不存在统计学意义(P>0.05);菌种具有较差的检出一致性,差异存在统计学意义(Kappa=0.262,P<0.01);NPA病毒检出率为较BALF更高;BALF与NPA在病毒感染中具有良好的HBo V检出一致性(Kappa=0.508,P<0.01)。结论下呼吸道感染中可将MP检测、NPA病毒作为重要参考依据,联合BALF能将呼吸道病原学检测敏感性提高。
Objective To analyze the application value of upper and lower airway aspirate in etiological diagnosis of lower respiratory tract infection in children. Methods From May 2010 to September 2017, 420 patients with lower respiratory tract infections at the Second People's Hospital of Luohe City were selected as study subjects.Collected the BALF(alveolar lavage fluid) and NPA(nasopharyngeal aspirate)and conducted multiple pathogen detection to analyze the combined effect of multiple pathogens. Results When detected with BALF and NPA, there were 386 patients were positive, and the positive rate was(91.9%). Compared with BALF, the detection rate of NPA was higher(P 0.05);.When detected bacteria with BALF and NPA,the total positive patients were 72 cases, accounting for(17.7%)Compared the total positive rate of two group,the difference was not statistically significant.(P 0.05). The bacterial strains had a poor detection consistency and the differences were statistically significant(Kappa=0.262, P 0.01).The detection rate of virus in BALF was higher than NPA. BALF and NPA had good HBo V detection consistency in virus infection(Kappa=0.508, P 0.01). Conclusion MP detection and virus in NPA can be used as important references in the lower respiratory tract infection. The methods of NPA combined with BALF can improve the sensitivity of respiratory etiology detection.
