基于患者肝肾功能指标开展地高辛个体化给药的临床应用考察研究 被引量:3

摘要 地高辛广泛应用于心源性疾病,但往往难以掌握确切剂量。本研究的目的为分析患者生理因素及肝肾功能相关指标对地高辛血药浓度水平的影响,为合理地应用地高辛提供理论参考。回顾性收集南京鼓楼医院2016~2017年口服应用地高辛的住院患者病历资料,聚焦肝肾功能相关指标,利用描述性统计方法对患者综合情况进行分析。按照纳入标准,共计收集256例应用地高辛的患者。相关性分析显示:患者体重、血糖、舒张压、尿素、肌酐、肾小球滤过率共计6项因素与地高辛的血药浓度值显著相关(P<0.05)。考察结果,患者的体重、血糖、舒张压、尿素、肾小球滤过率纳入了地高辛血药浓度的回归模型,且此回归模型用于地高辛血药浓度的预测具有临床意义。 Digoxin is widely used in cardiogenic diseases, but it is often difficult to grasp the exact dose. The objectives of this study were to investigate the effects of physiological factors and liver and kidney function related indexes on the serum concentration of digoxin, and aimed to provide theoretical reference for the rational use of digoxin. We collected data of 256 hospitalized patients who accepted oral digoxin for treatment in Nanjing Drum Tower Hospital from 2016 to 2017. Descriptive statistics were used to analyze the patients' comprehensive situation focusing on liver and kidney function indexes. Correlation analysis showed that 6 factors including body weight, blood glucose, diastolic blood pressure, urea, creatinine and glomerular filtration rate were significantly correlated with the serum concentration of digoxin(P0.05). Finally, body weight, blood glucose, diastolic blood pressure, urea and glomerular filtration rate were included in the regression model of digoxin serum concentration, and the regression model for the prediction of digoxin serum concentration has clinical significance.
