甲状腺乳头状癌中低密度脂蛋白受体相关蛋白1B的表达及意义

摘要 目的探讨低密度脂蛋白受体相关蛋白1B(lipoprotein receptor-related protein 1B,LRP1B)在甲状腺乳头状癌(papillary thyroid carcinoma,PTC)中的表达及其发生、发展中的作用。方法收集79例PTC、结节性甲状腺肿30例、癌旁正常甲状腺组织25例。应用免疫组化En Vision两步法检测各组低密度LRP1B蛋白的表达,并分析PTC中低密度LRP1B表达与临床病理特征的相关性。结果低密度LRP1B在PTC中的阳性率(38.0%)低于结节性甲状腺肿(73.3%)及正常甲状腺组织(80.0%,P均<0.01)。低密度LRP1B表达在PTC伴淋巴结转移组中低于不伴淋巴结转移组(P<0.01)。单因素相关分析显示PTC组的低密度LRP1B表达与患者淋巴结转移和临床分期相关(P均<0.01),低密度LRP1B表达与患者性别、年龄、部位、肿瘤大小、TG-Ab值和TPO-Ab值无关(P均>0.05)。经多元逐步回归分析发现在PTC组的低密度LRP1B与淋巴结转移呈正相关。结论低密度LRP1B可能参与PTC的发生、发展和淋巴结转移,提示其有望成为甲状腺肿瘤良恶性诊断和早期淋巴结转移风险评估的生物学因子。 Purpose To investigate the expression of low density lipoprotein receptor-related proteinl B ( LRP1 B) in papillary thyroid carcinoma and its role in the development of papillary thyroid carcinoma. Methods Seventy-nine cases of papillary thyroid carcinoma tissues, 30 cases of nodular goiter, and 25 cases of adjacent normal thyroid tissue were collected. The expression of low density LRP1B was detected by immunohistochemistry of EnVision method, and the relationship was analyzed between the expression and the elinicopathological features of papillary thyroid carcinoma. Results The expression of low density LRP1B in papillary thyroid cancer group (38.0%) were higher than that of nodular goiter group (73.3%), as well as the adjacent thyroid group (80. 0%, both P 〈 0. 01 ). The expression of low density LRP1B protein in papillary thyroid carcinoma with lymph node metastasis was higher than that in group without lymph node metastasis ( P 〈 0. 01 ). Univariate correlation analysis showed that the expression of low density LRP1B in papillary carcinoma was correlated with lymph node metastasis and clinical stage (both P 〈 0.01 ). Low density LRP1B protein had no relation with gender, age, location, tumor size, TG-Ab value and TPO-Ab value ( all P 〉 0. 05 ). Multiple stepwise regression showed that there was a positive correlation between low density LRP1B and lymph node metastasis in papillary carcinoma. Conclusion Low density LRP1B may be involved in the development of papillary thyroid carcinoma and lymph node metastasis, suggesting that low density LRP1 B may be an important biological factor in the diagnosis of benign and malignant thyroid neoplasms as well as assessment of early lymph node metastases.
